Our Mission
Dominican Sisters Family Health Service, a nonprofit home
health agency, provides compassionate, comprehensive and
family focused home care to persons in need; including the
poor, marginalized, and the elderly. As a community-based,
certified agency, special emphasis is given to the health
and social needs of the community and its members. Our
programs and services promote health, independence, and
quality of life.

Dear Friends,
Looking back at 2015, we at DSFHS are proud that we provided quality
home care in the face of an ever-changing, complex healthcare landscape.
It hasn’t always been easy. So much is changing in the world around us.
Demands are increasing, resources are being stretched, and we are
continually called on to do more with less. However, our mission and our
commitment to providing quality, compassionate care is unwavering.
In 2015 we continued to build and strengthen alliances. We held leadership
roles with the provider network for the State Medicaid redesign program
with partners including Westchester Medical Center, Bronx-Lebanon
Hospital Center and Stony Brook Medical Center. We further developed
partnerships with area hospitals in response to Medicare’s new model to
support quality and care improvements for joint replacement patients’
transitions from surgery to recovery.
As we worked to achieve better health outcomes at lower costs, we still
accomplished so much. We increased our focus on acutely and chronically
ill children. We developed our palliative care services and continued our
pastoral care counseling. We completed our second year of an innovative
collaboration with Stony Brook School of Medicine, pairing medical students
with clinicians. And, we rolled out a telehealth program as part of our care
continuum serving the Shinnecock Indian Nation in Southampton.
Despite the fundamental changes taking place in the healthcare industry,
we provided services to those in our communities who needed them most.
We served vulnerable and high-risk community residents through our
community-based programs in New York City and Suffolk County.
Thanks to our dedicated staff and Board, we will continue to provide
the highest quality care to all. As DSFHS transitions to our new Catholic
affiliation with Archcare, we thank our long-standing sponsors, Dominican
Sisters of Blauvelt, Hope, Sparkill and Franciscan Sisters of Allegany.
As we move forward, we will continue to lead, giving those we serve the
care and peace of mind they deserve. May God bless you for your support

2015 DSFHS Highlights
• Named to prestigious Top 500 of HomeCare Elite in the country
• Launched Palliative Care Program for patients and their families facing chronic and serious illness

• Our fall 2015 Hope Gala was a resounding success. Nearly 200 people - both old friends and new
ones - joined us to bring vital support for DSFHS and our pastoral care program.
• Our annual spring Wines Around the World fundraiser brought in critical funds for our community programs.
• Our three Eastern Long Island events included a spring High Tea, summer Cocktails at Sunset and annual
fall Golf Outing, which all benefitted the Helping Hands program.

8 year-old Nataya has multiple medical conditions including heart problems,
developmental delays, abnormal gait and asthma. DSFHS provides Nataya with
skilled nursing, social work, and speech and physical therapy services, enabling
her to live at home.

DSFHS Serving the Community
In the South Bronx, a community challenged by the problems associated
with poverty, DSFHS provides a range of services. Through our
Comprehensive HIV/AIDS Services, funded by Ryan White Part D,
DSFHS provides medical and supportive services to HIV-positive women,
children and youth and their affected family members. We offer HIV and
Hepatitis C Rapid Testing and Counseling at our Bronx location and at
other community-based organizations with support from New York City
Health and Hospitals Corporation. Our Community Walk-in Center for
Health and Wellness provides a wide range of social services, workshops
and referrals to partner agencies in the South Bronx community. We served
more than 1,400 clients in 2015.
Family Life, funded by the New York City Administration for Children’s
Services, provides case management and workshops for at-risk families.
In Suffolk County, the Helping Hands Program, with support from the
towns of Riverhead and Southampton, provides non-medical support to
elderly or disabled people living in these towns Our Expanded In-Home
Services Program for the Elderly (EISEP), funded by the Suffolk County
Office of Aging, provides non-medical support services for residents of
Southampton who are 60 or older.
Our three Thrift Shops, in Greenport, Hampton Bays and Sag Harbor,
generate additional revenue for the Helping Hands Program.
Speaking about our pastoral care counseling, D. D. was an
independent, energetic 80 year-old. After a severe bout of
pneumonia, she came home from the hospital. DSFHS
provided a nurse and physical therapist to help her
convalesce at home, but she was very weak and her morale
was low. “I was alone and felt hopeless. I didn’t know
I had a spiritual life until you visited and continued to
stay with me throughout my recovery.” ~ D.D.
