4. Payment Details
I enclose a cheque made payable to IoS for £69.00 (Single IoS), £84.00 (single IoS & RAPs), £50.50 (Joint IoS), £65.50
(Joint IoS & RAPS).
I will call you to make payment for my membership – I understand that until payment is made I will not be covered under
the insurance. Please leave at least 48 hours before calling.
I would like to sign up to the IoS Direct Debit Scheme to pay my membership and I have completed the Direct Debit Instruction
overleaf. I understand that I will receive a letter with a Direct Debit guarantee to confirm my instruction.

Please note: It is a requirement of your IoS membership that you hold a valid DBS if you are teaching under 18s. The ASA requires
DBS checks to be carried out every three years, unless you have subscribed to the Update Service.
