I still remember how thrilled I was to find a place to work that combined my love of patient care with my passion for public health and the desire to work with people from many different cultural backgrounds. I had worked in Central America, Haiti and Africa and thought I was destined to work overseas. However, my wise mother pointed out that I could do the work I desired right here close to home. Home is now how I see TCC. Home is how our patients see TCC. A “medical home” where our caring staff is there for them, not only for their acute medical problems, but there for the crises they may face in their lives, the diseases they are trying to control or the destructive lifestyles they are trying to change. The staff is always there for our patients. We see ourselves not only as health care providers but as healers, partners and mentors. For so many of the children and youth we care for, we are the only college educated person they have known on a one-on-one basis over the course of many years. We all encourage them to do their best in school, lead healthy lifestyles and we celebrate their successes. My heart sings when my patients I have followed for over a decade bring in their high school awards received upon graduation. Our staff celebrates every patient who enters college, every patient who follows their dream, and every patient who is finally able to lose the needed weight or who is able to now control their asthma or diabetes. 2 Caring is what we are all about. I hope this will be obvious as you read through this biannual report; caring with the provision of the highest quality of care and innovative programs in health promotion and chronic disease management. We could not do what we do if it were not for our incredible staff, amazing board of directors led by Victor Gonzales, our partners in our many collaborative efforts and especially the donors that support us to do our work. It is my pleasure on behalf of the Board of Directors to share with you some of those accomplishments in this 2006 – 2008 Annual Report. It has been said numerous times: in giving, we receive. As we thank all of our 2006 – 2008 supporters of The Children’s Clinic, we hope that you feel you have received much from your generous gift. In addition, many thanks go out to Dr. Elisa Nicholas for her leadership, the hardworking staff at The Children’s Clinic and the numerous volunteers whose efforts touch many families in our community. Significant challenges are on the horizon for all non-profits as a result of the current state of our economy. The Children’s Clinic is no different and, in fact, in times like this the clinic and its services become even more important. Recordbreaking visits were provided in the most recent fiscal year giving us the ability to provide to families that have nowhere else to turn. 3 Watch Us Grow The Children's Clinic continues to grow and establish new programs to better serve children and their families in the greater Long Beach and South Bay area. Some of the 2006-2008 accomplishments have been highlighted below to show the essential role the clinic has played in the health care of the community. Did you know? TCC provides free walk-in immunizations for any child in the community meeting poverty and insurance guidelines. In FY 07/08, TCC provided 21,110 immunizations! “The thought of what our community would be like without TCC is incomprehensible – the 18,000 patients – where would they go? On a recent visit, Angelina’s father, George, discussed with the doctor the MRI procedure Angelina will have to undergo to determine the cause of her optic nerve atrophy condition. The Children’s Clinic staff helped George and his wife Song access coverage for Angelina under California’s Healthy Families program, which provides low cost insurance for children who do not have coverage and do not qualify for free Medi-Cal, otherwise her condition may not have been diagnosed and treated. George is a utility contractor and work is difficult to come by right now, so he has taken on additional parenting duties while Angelina’s mom works full time. TCC serves as a medical home, which is very important to pediatrics. We approach all patients with respect, go the extra mile and ask the extra questions, so that all of their concerns, and ours, are addressed. Since almost half of all Americans live with a chronic condition, and half of those have multiple chronic conditions, much of our care and quality improvement projects focus on asthma and diabetes as well as heart disease, high blood pressure, obesity and depression, many of which occur together. Wise, Esq., TCC Board Member 6 Helping Children Breathe Easier Meet Latrice Latrice is a precocious three year old, with all the makings of a child star, but behind her beautiful smile there are worries. Latrice suffers from chronic asthma and obstructive sleep apnea. She has been rushed to the emergency room seven times in her short three years before her daughter’s god mother, Shaquanda, told mom to take Latrice to The Children’s Clinic to see Dr. Nicholas. Latrice’s mom, Lashonna, wasn’t sure what to expect. She had taken Latrice to several doctors and with each visit a different diagnosis. Upon meeting Dr. Nicholas for the first time, Lashonna recalls the feeling of “coming home”. “I felt as if someone was going to finally tell me what was wrong with my little girl,” she recalls. Comprehensive testing was the first step in determining the severity of Latrice’s asthma. Once Dr. Nicholas explained the treatment options she referred Latrice and her mom to Dr. Terry Chin, PhD, Associate Director, Miller Children's Hospital, for further asthma treatment and to TCC's Asthma Community Health Worker, Carolina Sanchez. Carolina visited the family at their home, through the Home Visiting Program, and provided information, resources and training on asthma care including decreasing in home asthma triggers, help with organizing Latrice’s asthma medication schedule, referrals and appointments. Carolina spent two hours with family on their first visit and provided instruction and information on how to care for asthma. Lashonna said of the first visit, “I wanted to cry. I was so grateful to have someone come to my home and help me to understand my daughter’s asthma and how to take better care of her.” Today, Latrice’s asthma is under control and steps are being taken to resolve the obstructive sleep apnea. All Latrice knows is that she’s her mom’s little star and that’s the way it should be. For more information about TCC’s Asthma programs, please contact Leslie Larsen, CPNP at (562) 933-0497. • According to the Centers for Disease Control (CDC) asthma is a leading chronic illness among children and youth in the United States. • In 2006, 5.6 million school-aged children and youth were reported to currently have asthma and 3.1 million had an asthma episode or attack within the previous year. • Our Asthma Care Team is a multi-disciplinary disease management team made up of TCC Administrators, Health Care Providers, Medical Assistants, Nurses, Health Educators, and Social Workers. • Throughout FY 06/08 TCC continued to serve a leadership role in two crucial programs which target childhood asthma in the Long Beach community: the Long Beach Alliance for Children with Asthma (LBACA) and the Physician Asthma Care Education (PACE) Community Health Worker Home Visiting Program. 7 Diabetes: Focus On Prevention And Management Diabetes affects nearly 24 million people in the United States, an increase of more than 3 million in approximately two years, according to 2007 prevalence data estimates released by the Centers for Disease Control and Prevention (CDC). This means that nearly 8 percent of the U.S. population has diabetes*. At The Children’s Clinic Family Health Center at Cesar Chavez Elementary School, Dr. Peter Yeh estimates about half of the adult patients he sees are afflicted with diabetes. One patient, a 33 year old man, is his “superstar” patient. The man didn’t realize he was “at risk” for diabetes until he participated in a workshop presented by the Long Beach Diabetes Collaborative. Afterwards, he made an appointment with Dr. Yeh and since that time, he has been a model patient, following the treatment plan and making healthy decisions about his nutrition. As a result his diabetes is well under control. At The Vasek Polak Children’s Clinic Family Health Center on Pine Avenue, Dr. Patricia Bellas, who has been with TCC for 7 years and a practicing physician for over 20 years, estimates that she has about 300 active patients who have been diagnosed with diabetes. She says the key to combating this disease is by “taking control and owning it”. Self-management plays a large part in this and Dr. Bellas and Dr. Yeh both take the time to work with patients and families. By building trust and looking at patients in a comprehensive way, they and all the other TCC doctors, providers, nurses, educators and medical assisants are working to combat diabetes one patient at a time. For several decades, TCC has played a major role in the care of children and adults with diabetes. Our diabetes programs continue to be nationally recognized for their focus on prevention and management of this chronic disease. Our approach is simple, yet very effective. We provide a multidisciplinary approach including social support and nutritional counseling and involve the entire family making a lasting change in the home environment. Atlanta, GA: U.S. Department of Health and Human Services, Centers for Disease Control and Prevention, 2008. 8 Events: Grand Re-Opening Of The Main Clinic At Buffum Pavilion On Thursday, May 24, 2007, TCC celebrated the grand opening of the Main TCC Clinic to the Buffum Pavilion, adjacent to the Long Beach Memorial Medical Center Emergency Room and Miller Children’s Hospital. Los Angeles County Supervisor, Don Knabe, Miller Children’s Hospital, Long Beach Memorial Medical Center leadership and TCC Board Members were on hand to cut the ribbon. Entertainment was provided by the Ballet Folklorico Cultural Mexicana children’s dance troupe of the Community Partnership Council. The new Buffum Clinic is increasing access for the growing population by offering additional patient visits. Care Health Plan The Boeing Employees Community Fund 7 Events: Charity Regatta In 1987, four local yacht club commodores got together and decided it was time to give back to the community. Pete Ives, Long Beach Yacht Club; Hans Schoedl, Shoreline Yacht Club; Tom Ramsey, Alamitos Bay Yacht Club and Larry Lewis, Seal Beach Yacht Club had several meetings at each other’s homes to set the plan in motion that for 22 years has benefited The Children’s Clinic. Since that time, four more yacht clubs have joined the team; Cerritos Bahia Yacht Club, Little Ships Fleet, Long Beach Singles Yacht Club and Navy Yacht Club of Long Beach. Whether the winds are topping out at 8 knots or 16 knots, come September, sailboats will head out into the bay for a day of racing in support of TCC. On September 24, 2006, the Charity Regatta garnered more than $34,000 in donations from racers and sponsors while in 2007 more than $40,000 was raised. TCC is extremely grateful for all the hard work and effort that goes into planning, promoting and executing this event in support of the children and families TCC serves. Meet Brian The Children’s Clinic (TCC) is proud to be the community’s safety net, providing health care services to families in need. At the Burger Bash following last year’s Charity Regatta, Dr. Maria Chandler, TCC’s Chief Medical Officer, was approached by a family attending the festivities. The mother asked Dr. Chandler if she remembered their teenage son Brian, and informed her that she followed him at the clinic for most of his childhood. This was a working family without health insurance, and having no other options for care, the children were able to see a pediatrician and get needed health services at TCC. When the family was finally able to get private health insurance, Dr. Chandler referred them to a local pediatrician who was trained as a resident at TCC. TCC’s resident angel educates, mentors and protects and has been doing so for over 20 years as one of TCC’s Pediatric Nurse Practioners. Through her work at California State University, Long Beach, UCLA, and Mount St. Mary’s she has instructed aspiring nursing students, while at TCC she has educated young mothers to be on how to best care for their infants and children. Her friend and TCC colleague, Leslie Larsen writes, “Lynn is an amazing health care professional – experienced, capable, compassionate, (and) willing to enter into the lives of her patients and their families and truly care about them.” Lynn is a natural mentor, empathetic and kind, exemplifying all that is best about TCC and our Teamwork of Culture and Caring. She has touched the lives of many a new nurse, pediatrician, resident, medical assistant and staff. Lynn is a “true angel”, says Dr. Elisa Nicholas. She cares and protects her friends in times of need and does the same for the children and families she sees. She believes in the children and families she serves and is “one of the clinic’s strongest advocates in promoting both the physical and psychosocial health of the people the clinic serves,” said Margaret Brady, PhD, RN, CPNP of California State University Long Beach. Lynn will be retiring in the fall of 2009, but that doesn’t mean she’ll be far from TCC. Retirement presents new challenges and new opportunities to effect change in the community. 14 I love Thursdays and according to my family, I am always happier on Thursdays. It must be because it is the one day a week that I exclusively see patients and their families. As a pediatrician for the last 20 years, I find no greater fulfillment than in being allowed to share the challenges, joys and triumphs of my patients and their families. Ultimately though, to be able to combine patient care, administration, program design and public health advocacy is truly a gift.
