Dear Parents,
The purpose of this letter is to make you aware of changes in the immunization rules regarding
rising 7 grade and rising Kindergarten students for the upcoming 2016-2017 school year. The
following information has been provided by the North Carolina Department of Health and Human
Services.

Effective July 1 , 2015 all students beginning Kindergarten and 7 grade will be required to adhere
to the following changes:

Kindergarten:
 Polio vaccine – the booster (4 ) dose is required on or after the 4 birthday and before
entering school for the first time.
 Varicella vaccine – 2 doses administered at least 28 days apart.
 Second dose before entering school for the first time.
 Documentation of disease must be from a physician, nurse practitioner, or
physician’s assistant verifying history of varicella disease.

Seventh Grade:
 Tetanus, diphtheria and pertussis (whooping cough) – Tdap
 Booster dose of Tdap for individuals who have not previously received it and are
entering 7 grade or by 12 years of age, whichever comes first.
 Meningoccoccal conjugate vaccine (MCV) - 1 dose prior to entering 7 grade or by 12
years of age whichever comes first.

Providence Day School requires all new students provide an updated Immunization Record prior to
the first day of school. If you are a parent of a rising Kindergartener or 7 grader, please make sure
your child’s physician has given you the most up to date record.
