Elementary Teacher Preparation Program (K-8) Field Instructor Mid – Year Feedback Form Dear Interns, Field instructors play an important instructional role in supporting your professional learning during your internship year. As with any teaching and learning situation, specific and constructive feedback from interns about how field instructors are fulfilling their roles and responsibilities‐‐given the time and resources available‐‐can help them understand their strengths and areas that need further work in order to be more responsive to your professional learning needs. Field instructors can also use this feedback to help the Teacher Preparation Program grow and develop in a positive direction. Your field instructor will ask for your written feedback two times this year‐‐now, and at the end of the year. The mid‐year feedback will be used to identify areas for improvement in field instruction for lead teaching and bringing the internship year to a close. The end‐of‐year feedback will be used to identify areas for improvement in field instruction for the future and will be valuable information for ongoing program improvement. For each category below, please use the rating scale to indicate how helpful the support you received from your field instructor during fall semester was in supporting you in learning to teach. A rating of 4 means the support you have received so far this year has been "most helpful in learning to teach."
