Please return the following items in a single gallon size
zip lock storage bag clearly marked in permanent ink
with student’s name:
2 granola bars
2 small boxes of raisins
1 pack cheese and crackers
2 pull top fruits or meats w/plastic spoon
2 small juices (pull top cans or plastic bottles-NO juice boxes)
2 small bottles of water
2 large garbage bags
1 small toy or comfort item
Please do not include any items that need cooking or refrigeration. No glass or combustibles. Everything must fit inside ONE gallon size zip lock bag. If you would like to prepare an extra bag for a student who needs one, please feel
free to do so and mark it EXTRA. Kits will be stored in your
student‟s classroom.
Emergency Kit Information:
A primary concern of our school staff is the safety and well
being of our children. In the event a major natural disaster,
extended lockdown, or other severe emergency occurs, all
students will be kept at school until they can be safely released. The school could be caring for your child(ren) for
several hours or more. We are asking each student at Lackamas to have his or her own bag of emergency supplies.
Please also remember to keep your contact information up
to date in the office.

**ASB Fee: Your student‟s $15 ASB Fee helps to provide many of the “extras” that wouldn‟t be possible out of
the building budget, such as special assemblies, live theater performances, student incentives (student of the
month), birthday pencils, field day, drama club, etc. Each classroom will also receive an activity budget from
this fee. Depending on the activity/field trip selected by the teacher, some classes may have an additional fee
necessary to participate. Please pay your student‟s ASB Fee at the beginning of the school year so that we can
plan all of these fun and special activities early! Checks should be made payable to “Lackamas Elementary”.
