Q. Under the FOIA please could I have:A copy of any guidance given to staff for completing T2 / T3 documents;
A copy of your Policy for Violence and aggression and / or, if separate, a copy of your policy
on the use of Rapid Tranquillisation.
In addition, I should be grateful if any answers to the below questions could be provided by
your pharmacy team (or other relevant specialist team).
1. Is your current Trust Policy for Violence and aggression – including pharmacological
management options (Rapid Tranquillisation) in line with the current NICE Guidance NG10:
Violence and aggression: short-term management in mental health, health and community
settings. Published May 2015?
2. Does your current Trust Policy for Violence and aggression (Rapid Tranquillisation) contain oral pharmacological treatment options compared to those detailed within the NICE
guidance NG10 ?
3. Does your current Trust Policy for Violence and aggression (Rapid Tranquillisation) contain alternative intra-muscular pharmacological treatment options to those detailed
within the NICE guidance NG10, for example midazolam i.m?
4. What intra-muscular product is being employed within your Trust when licenced
lorazepam injection is unavailable nationally – in line with your current Trust Policy for
Violence and aggression – including pharmacological management options (Rapid
Tranquillisation)?
5. Do pharmacy staff routinely check if physical observations as detailed within your
Trust’s current Trust Policy for Violence and aggression – including pharmacological
management options (Rapid Tranquillisation) are completed after oral medicines are
administered?
6. Do pharmacy staff routinely check if physical observations as detailed within your Trust’s
current Trust Policy for Violence and aggression – including pharmacological management
options (Rapid Tranquillisation) are completed after intra-muscular medicines are
administered?
7.Do your Trust specifically complete an incident form after medicines are administered for
Rapid Tranquillisation purposes – separate to the incident form completed for the actual
episode of violence / aggression?
8. Is this requirement specified within your current Trust Policy for Violence and aggression?
9. Is this for oral medicines, intra-muscular medicines or both?
10. Do pharmacy staff routinely check if capacity status has been checked and recorded in
line with the Mental Health Capacity Act when medicines are administered on inpatient
wards for all patients and all medicines?
11. Does your Trust have specific guidance in place for the completion of T2 and T3 forms?

This is not mandated in the policy

medicines are administered?
This is not mandated in the policy

Yes, this could be reported with 2 cause codes

Technically both, but in practice usually injectables

Yes
