Supplementary Product Disclosure Statement (SPDS) for: Allianz Workplace Benefits

This is an SPDS for the Allianz Life Plan Product
Disclosure Statement ('PDS') dated 29 October 2008.
Important Note: The benefits under this SPDS only
apply to participants in the Allianz Workplace Benefits
program.
Benefits of Accidental Death cover
In conjunction with your Allianz Life Plan policy we
are pleased to provide you with an additional FREE
$50,000 of Accidental Death cover in respect of each
Life Insured under your policy, subject to the terms
and conditions detailed in this SPDS.
You must read this SPDS together with the Allianz
Life Plan Product Disclosure Statement and Policy
Document – dated 29 October 2008.
Policy conditions
This SPDS contains the following:
• the Policy Schedule together with any Addendum
Policy Schedule that we send you if your
application is accepted
• Details of the additional Free Accidental Death
cover for Allianz Workplace Benefits
• Additional "Glossary" terms.
How to read this Supplementary
Product Disclosure Statement
This document gives you a brief overview of the
Accidental Death cover for Allianz Workplace Benefits
and its policy conditions. It needs to be read in
conjunction with the Allianz Life Plan PDS dated
29 October 2008. If there is any inconsistency
between this SPDS and the PDS, this SPDS will override
the terms of the PDS to the extent of the inconsistency.
We have bolded words that have special meanings
within the terms and conditions of the Accidental
Death cover the first time they appear. Definitions for
all bolded words (other than those that appear as part
of headings) are in the Glossary section at the end
of this SPDS. In this SPDS references to ‘we’, ‘our’, ‘us’
and ‘Allianz’ are references to Allianz Australia Life
Insurance Limited. References to ‘you’ and ‘your’ are
references to the policy owner or life insured, as the
context requires.
Your policy with us is made up of:
• these Policy Conditions
• the Policy Conditions of the Allianz Life Plan as
detailed in the PDS dated 29 October 2008
• the Glossary.
A benefit listed below only applies if shown in your
Policy Schedule or Addendum Policy Schedule.
Benefits are only payable under the terms and
conditions of the policy. We will not pay a benefit if
an exclusion applies (see below). You must also
satisfy our claim requirements, explained on page 5
of the PDS.
Policy benefits
Accidental Death cover
Accidental Death cover will pay a lump sum benefit
of $50,000 in the event of your accidental death. This
Cover Amount is fixed for the life of your policy and is
not subject to the Inflation Proofing benefit described
on page 4 of the PDS.
When is the Accidental Death benefit
not payable? (policy exclusions)
We will not pay an Accidental Death benefit where
the condition is a result of a self-inflicted injury.
No benefit will be paid in any circumstances if the

Life Insured has not applied for the Allianz Life Plan
personally or where the application was completed on
behalf of another person.
Policy is the policy number listed on your Addendum
Policy Schedule which you will receive once you have
applied and been accepted for cover.
No benefit will be paid where we have agreed with
you a special term in respect of any cover under the
policy for that Life Insured that specifically excludes
the event or condition leading to the claim.
Any such Special Term will be agreed with you
before your policy is issued and will appear on your
Policy Schedule.
Policy Anniversary means the anniversary of the
Commencement Date of your policy.
For example, if we agree to exclude "Base Jumping" in
respect of any cover and you were killed in the course
of Base Jumping, we would not pay a benefit.
Premium calculation and payment
No premiums apply to Accidental Death cover for
Allianz Workplace Benefits. However, to maintain your
cover you need to pay your Allianz Life Plan premiums
within 30 days of the due date. We will cancel your
policy if you do not pay your premiums within this
period and you will no longer be covered from the date
when premiums were due.
When does cover start and end?
We will provide you with this cover from the
commencement date of your policy. Cover in respect
of a Life Insured under this benefit ends on the earliest
of the following:
Policy Owner means the person who applies and is
accepted for this policy and who is named in the
Policy Schedule. The Policy Owner is the person who is
entitled to receive benefits under the policy and is the
only person who may extend, vary, cancel or otherwise
exercise any rights under the policy.
Policy Schedule means the document we send to you
titled "Policy Schedule" which sets out the details of
your particular policy including who is the Policy
Owner, who is the Life Insured, which benefits you
have applied for and been accepted for, any Special
Terms we have agreed with you, and your cover
Commencement and Expiry Dates.

a) the date the cover amount is paid in full by us for a
Life Insured
b) the date you cancel your policy or your policy
lapses due to non-payment of premium
c) the Expiry Date of your policy
d) Policy Anniversary after the 70th birthday of a
Life Insured.
Glossary
This Glossary is to be read in conjunction with the
Glossary Section on page 7 of the PDS.
Accidental Death means death as a result of sustaining
bodily injury caused by accidental, violent, external
and visible means while your policy is current.
Commencement Date means the date you first take out
your policy (this date will be set out on your Allianz
Life Plan Policy Schedule).
Expiry Date is the date at which cover ceases.
The expiry date for your Accidental Death cover will
be set out in your Addendum Policy Schedule.

Life Insured means the person whose circumstances
we assess and accept as a life insured and who
is named as such in your Policy Schedule and
Addendum Policy Schedule.
