Tele-interview Guide Your guide to completing the Tele-interview Thank you for choosing Bank of Ireland Life for your protection needs. Putting financial protection in place is a very wise move and we’re looking forward to providing you with peace of mind and security for many years to come. Before we put you on cover we need to find out about your health and lifestyle. This will determine the terms of your cover. Occasionally a client’s health or lifestyle may require a higher premium and in a small minority of cases we may not be able to provide cover. It is important however that you find this out now, to avoid any confusion at claim stage. To help you with this part of the process, your Financial Adviser has arranged a Tele-interview, which is an integral part of the application for all of our clients. What is a Tele-Interview? One of our team of specialist nurses will call you to gather information about your health and medical history to help us determine the details of your policy. It is really important that the information you provide is correct and complete, as this will be used to determine if we can offer you a policy and the level of premium required to provide the right cover for you. If you don’t answer the questions truthfully or in full, it could mean that your policy may not pay out in the event of a claim. This is called non-disclosure. If you are unsure about something please let us know. Based on our experience we found that this is a more customer friendly way to complete your application as many clients prefer to discuss these details with a qualified person over the phone. But don’t worry, we will also send you a copy of the questions and answers afterwards so that you can check what you have said. If you find the answers are incomplete or inaccurate you are required to notify us within 10 working days. There is also an on-going requirement that you notify us of any changes to your details that may arise between the time of application and before the policy has commenced. Please allow approximately 30 minutes to complete the call. What sort of questions? To begin we will ask you to confirm your identity to make sure we are talking to the right person. The Tele-interviewer will then ask questions on your occupation, medical information and hobbies that are relevant to your application. What do I need to prepare? It is useful if you take a little time to prepare for the Tele-interview. We aim to get through all answers on the call so it will speed up the process if you have the following information to hand: 1. Medical information • Your doctor(s) name and address • Your medical history including dates and results of any tests • Names and dosage of any medication you are taking • Your family medical history (mother, father, brothers, sisters) • Height and weight details 2. Occupation details We may ask some detailed questions about your work, for example, whether you work at heights. 3. Overseas travel We may ask about trips overseas, countries visited, how often and for how long. 4. Sports and hobbies If you take part in dangerous pursuits, such as motor sports or scuba diving we will need to know full details, including how often and your level of experience. What happens after the call? After we complete your Tele-interview we will send you a written copy of your report detailing the questions we asked and your answers. Please check the report carefully to make sure the information you have provided is correct. If you notice something is incorrect or that information is missing then you must write in any corrections and return the report to us within 10 working days. Please sign and date the amended report. In some cases we may require additional information from your doctor. If that happens your Financial Adviser will let you know. If your application is accepted Once we have the output from your Tele-interview and any additional medical information, you could be accepted straight away. If this is the case we will let your Financial Adviser know. If we can’t cover you straight away Every case is different. Sometimes medical evidence may highlight that it is necessary to revise terms of the policy. If we are unable to offer cover at normal rates we will provide you with details of the cover available and/or a revised premium. This is important to ensure we have you on the correct cover. If this happens we will notify you and your Financial Adviser.
