Current Resources
• CCG has inherited 16 contracts/agreements
with the sector
• Many of the agreements are old with poorly
defined outcomes, & KPIs
• Total Value approx £200,000 pa
• Most are countywide
Priorities 2013/14
 Dementia – early diagnosis and better support for
patients and carers
 Orthopaedics – Oxford Knee Score
 Reducing Unplanned admissions – better care at home
 Physical Activity, Healthy Eating and Weight
Management
 Supporting independence at home, better supporting
people with long term conditions, and addressing the
needs of adult and young carers.
 Creation of good developmental and learning outcomes
for children and young people
