How to get the greatest impact? Who are the people who need it most? • Prevention especially emergency hospital admissions and reaching social services eligibility How can the voluntary & third sector help us innovate to do this? What does a good outcome look like? Current Resources NHS Norfolk supported: • volunteering infrastructure to underpin health • Third sector projects e.g. • • • • • • • • Education programmes Cardiac rehabilitation Discharge support Equipment loans Young carers support Counselling Transport Hospice care Very useful projects but no strategic co-ordination or integration Current Resources • CCG has inherited 16 contracts/agreements with the sector • Many of the agreements are old with poorly defined outcomes, & KPIs • Total Value approx £200,000 pa • Most are countywide Priorities 2013/14 Dementia – early diagnosis and better support for patients and carers Orthopaedics – Oxford Knee Score Reducing Unplanned admissions – better care at home Physical Activity, Healthy Eating and Weight Management Supporting independence at home, better supporting people with long term conditions, and addressing the needs of adult and young carers. Creation of good developmental and learning outcomes for children and young people Plan on a page We are developing: A local vision on volunteering A local strategic direction for how we commission from voluntary & third sector partners Local solutions vs countywide collaboration? What outcomes do we want for local people who are supported by the voluntary & third sector? What should the process be for allocating resources?
