Dear Sir/Madam, You are requested to take following additional precautions as per our various pervious Circulars. Account Opening Form Bank Statement as Proof of Address: a computer-generated statement without signature/stamp of the bank (showing the name and address of the BO and bank transactions during a period, printed on the Bank stationery) can be accepted as a valid proof of address only if duly attested by the concerned Bank under the stamp and signature of the bank official. OR Bank Statement as Proof of Address: Original bank statement (showing the name of the BO, address of the BO and bank transactions during a period, printed on the bank stationery) self attested by the BO along with cancelled cheque (in original) pertaining to the said bank account, without bank attestation as mentioned above, provided that such original bank statement is not more than 2 quarters old can be accepted as valid proof of address. On proof if any additions or corrections Like Account number / MICR number / IFSC Code changed on Bank Passbook or Bank Statement it should be authenticated by the concerned Bank Officer under the stamp and signature of the bank (i.e. Bank Manager). No round stamp with short initial is allowed for such authentication. Correspondence Address Address of Third party will be accepted as a correspondence address, only if BO is providing all details as prescribed in ‘Know Your Client’ norms along with Account Opening Forms.(Proof of identity and Proof of address for the third party should be obtained). Nature of Business is mandatory to fill Account Opening It has been observed sometimes BO signatures are differ on all proofs and on Account Opening form, Agreement so please avoid the same. In case of any Correction, Overwriting, Cancellation, etc do not use “WHITE-INK” just cancel it with 2 simple lines like /----------/ on the wrong details and filled up the new details along with signature of all the account holders or else Fill New Form Account Opening for Partnership Firm Account Partnership Firm Account BO Account can be opened in the name of partner(s). BO accounts in the name of the Partnership Firm cannot be opened in CDSL system. Following additions documents are required Authorised signatories list with specimen signatures Copy of the Balance Sheets for the last 2 financial years (to be submitted every year). Registration Certificate Account Opening for Corporate Account Copy of the Balance Sheets for the last 2 financial years (to be submitted every year). Copy of latest share holding pattern including list of all those holding control, either directly or indirectly, in the company in terms of SEBI takeover Regulations, duly certified by the company secretary/Whole time director/MD (to be submitted every year). DIN Numbers of Directors Account Modification Form For Signature Modification please mention the Reason for updating/ changing the Signature, Bank Attestation should be done by the Bank registered with the Demat Account and proof of identity (PAN CARD copy self attested by BO with IPV sign & Stamp on it should be enclosed with Signature Modification Form) Delivery Instruction Slips 1. Following points are to be noted before submitting the slips for execution. a. On the top mark tick Delivery column/write Date and Name BO b. ISIN No. / Securities Name / Qty in figures/words and Total No. of instructions to be mention on slip. c. Off market transactions with no consideration - Reason to be marked. If Reason is “Others” explanation required in details. d. Counter BO-name/CM-name/CMBP-Id need to be mentioned. e. The BO must strike of the unused blank rows on the instruction slip with two thick firm lines as to prevent misuse of the same. 2. Originals for instructions sent by mail/Fax have to be sent within 2 working days failing which instructions by mail/Fax may not be entertained in future. 3. In case of any incomplete slip as per above which are execute by Fax / mail, AGAIN new Fax/ Mail required for execution of slips. Account Closure 1. In Account Closure Form please mention “Reason for Account Closing”. 2. Provide 2 Client Master Report with Logo of Depository (NSDL) (One Original and other Xerox Copy) duly signed & stamp by official of the new DP. Demat Request Form 1. Provide 2 copies (One Original and other Xerox Copy) duly signed DRF, Remat, Destate, Transmission Form, Transposition Form and Death Certificate, etc. 2. In Demat Rejection for “Signature Mismatch” Bank Attestation should be done by the Bank registered with the Demat Account 3. While rectifying / resubmitting Demat Rejection provide copy of RTA Rejection Memo along with rest of the documents as mentioned in the memo.
