General Membership Meeting Minutes
November 19th, 2015

Introductions: The group introduced themselves.
Minutes - September 2015: The group agreed to table approval of the September 2015 meeting
minutes due to a large number of attendees not seeing them. HSD will redistribute the meeting
minutes from September and the group will approve them at the next General Membership meeting.
Treasurer's Report/HSD Report: Diane shared that the payment process was recently transferred to
HSD. Derrik, from HSD, reviewed the current financial status. Current obligations are $26,904.89,
accounts receivable are $29,583.55, and net assets are $2,678.98.
CHIP Committee Updates/ACHEIVE Report: Hillary gave report on the Suicide Prevention Task
Force (SPTF), Healthy Communities That Care (HCTC), and Access to Care Committee. The SPTF is
currently showing a movie theatre ad (Derrik played the ad for the group), are continuing to compile
toolkits for law enforcement and plan a law enforcement in-service for December. HCTC is
continuing to work on placing prescription drop boxes in the county and continues education around
prescription misuses. Jenn shared that 550 pounds of medications were collected from the prescription
drop box at the Sherriff's Office throughout 2015. Access to Care Committee is in the middle of a
media campaign for open enrollment (bus ads and billboards), they are also promoting their various
tools, and are working on creating a preventive care fact sheet. Lisa reported on the Chronic Disease
Committee. The Baby Cafe continues to meet with 4-5 moms per session. The committee recently
completed a post survey for their initiatives around stroke. They are working on need to compiling the
data. Patty shared that recent data just came out through the state and a large percentage of people
know very little about strokes. She shared that Noyes is having a presentation about stroke at the
hospital on 12/03. She also shared that stroke is the 4th and 6th leading cause of death (US, NYS).
Lisa shared that they committee adopted new guidance for their restaurant initiative (8 restaurants
participating). They are still working with 2 grocery stores for their healthy retail initiative, and
Geneseo completed a School Health Index in the summer. The Committee is also working on a "Know
Your Numbers" campaign for the new year. Lisa also reported on ACHIEVE. They are working on
their third municipality for their trail initiative (Geneseo Highland Park fitness path with fitness
stations). They are extending the survey for the trail until the end of the month. The group agreed to
have Liz wait to give her update on the Safe Communities Committee because she will be presenting
later in the meeting.

Nominations Committee: Pam Hill is chair of the Nominations Committee, she was unable to attend
the meeting. Sue and tom reported out on behalf of the Committee. They were very busy over the last
month, recruiting nominations. Sue shared that the Committee will continue to identify additional
members and solicit nominations, they will continue to follow up with contacts over the next year.
There are up to 13 slots available on the Board. Please let Sue, Tom or Pam know if you think of any
contacts. Jenn suggested reaching out to law enforcement. Sue shared that they were able to get in
touch with Kim Hess from Monroe Plan, she may have someone to represent them on the Board. Sue
was hopeful that we may have this representative for the next General Membership meeting. The
Committee is also working on a spreadsheet to track each members terms. Sue provided the slate
below for a vote.

Sue motioned and Tom moved, all were in favor. HSD will update distribution lists, etc. to include the
new Board members.
CHA/CHIP Update: Jenn spoke about the Community Health Assessment/Community Health
Improvement Plan update. The current CHA/CHIP was completed in 2013. The state now wants the
hospitals and PH to be on same cycle with their assessments/plans. The update needs to be completed
by December 2016. The plan will run through 2018. Part of the assessment will be completed through
the S2AY Rural Health Network, a regional collaborative. Livingston County was not part of S2AY
during the last assessment, so this will be very helpful. S2AY will also assist with doing a crosswalk to
make sure that the assessment meets all requirements of the various partners (accreditation for Public
Health, the hospital, the IRS, etc.). Jenn shared that they would like to engage the community more in
this assessment. Lisa then gave a short presentation on the MAPP process. She passed out a fact sheet
on the MAPP process and information on the community themes and strengths section. There are four
parts to the MAPP process. These four parts are the Community Health Status (completed by
FLHSA/S2AY - shooting for February), Local Public Health System Assessment (completed by S2AY
- shooting for March), Community Themes and Strengths (completed by GVHP - shooting for April),
and Forces of Change (completed by S2AY - shooting for May). After all four parts have been
completed, the results are compiled and priorities are chosen. Lisa shared that they have been thinking
about using the Taste of Livingston County (TOLC) as a venue to hold a community discussion for the
Community Themes and Strengths section.
The group then had a discussion about how to engage the community more/what venues may be good
for focus groups, etc. Liz thought that it may be helpful to tie a survey into the website or the
HelpSource (when someone clicks on it the survey pops up). She also shared that we may be able to
reach parents through the schools, they have distribution lists that go out to all parents. Many thought
that it would be helpful to use existing groups to solicit feedback. It was mentioned that we could
utilize inmates, this is a different demographic to reach. Several felt that they best way would be to go
to places where people already are (events, TOLC, etc.). Sue shared that congregate meal sites would
be a good venue, they are well attended (and there are 7 different senior clubs we could reach). Sue
shared that she just finished surveys with aging population , we could use that data. If you know of
data sources that could help this project, please let us know. The Planning Department is also doing an
assessment right now, they are going to have a survey tool in the spring that will have some data we
could use. It was also mentioned that there are four major grocery stores in the county, we could

utilize college students to conduct surveys outside of them for a weekend. Jenn shared that they also
want to share with the community what is being done ("photovoice" section), so that it's not just them
talking. This could be a good way to engage college students. She shared that it would be great if we
could make the TOLC free, to reach more of the disparity populations, who really need the education
on healthy eating, rather than the more well off. Liz shared that hundreds of people attend the
backpack distribution and they are the people we want to reach. She suggested doing a clothing or
sneaker drive in the spring, this could draw in a lot of the population we want. The group discussed
ways of reaching the disabled and Hispanic populations. Suggestions included Literacy Volunteers,
Catholic Charities, and the Migrant Center. Catholic Charities shared that they could do small focus
groups with their parenting classes (they have some in the jails) and can survey people that come into
their office too. The group talked about distributing surveys and focus group questions to the whole
HealthFlash list, then everyone can gather the information as they meet with clients, hold groups, etc.
Ryan Shared that the Annual Job Fair in April usually has 200-300 attendees, they could easily have
them complete surveys. The group also talked about engaging Lori McEvoy, she could do facilitate a
group with Hand in Hand and possibly the Coalition of Churches. The CHA/CHIP update Will be a
standing agenda item for all of the meetings in 2016. Jenn, Patty, and Lisa will work together to create
a survey template to be shared at the next meeting.
Safe Communities Committee Presentation: Liz gave a presentation on what the Safe Communities
Committee has been up to for the year. Their focus areas included fall prevention, antibullying/violence prevention, domestic violence, traffic safety, child abuse, and parenting education.
They group held a fall prevention event in September. The event was well received. The group is
looking for a larger venue for next year. Efforts for National Bullying Prevention Month were
expanded in Mt. Morris, Avon, and York this year. The Committee worked with Cornell Cooperative
Extension (CCE) on their helmets for heroes and Carfit events. They also conducted their Main Street
Goes Purple campaign for the second year (50-60 businesses participating between Geneseo and Mt.
Morris). The Committee also supported a curriculum around child abuse through RESTORE last
spring. The Committee has recently starting talking about parenting resources/education in the county
and would like to expand this initiative for next year's workplan. They would like to help fill the gap
at CCE (Donna Horton fully retiring at the end of December). Liz shared that the Committee would
like to reach out to the partnership to make sure that we are covering all resources for parenting . The
Committee is going to develop two toolkits, one for parents and one for providers. Please send Liz and
questions or thoughts you may have.
Reports/Updates from Membership:
Liz (Dept. of Health/GVHP) - For violence prevention, she is working on the next United Way (UW)
grant. Last year GVHP was awarded $8,000 in support of her efforts. This was more than the year
before, throughout this year she has been able to expand the curriculum and materials. She shared that
she gave a presentation to National Grid in the morning. They have had 100% participation in UW for
the last 10 years. She is continuing to work on updating her toolkits and materials , the Dignity for All
Students Act is constantly changing so there is always information to update.
Lisa (Dept. of Health) - She shared that the Dept. of Health is offering flu shots at their immunization
clinics on the first Tuesday of every month. She also shared that they have several copies of the
Maternal Infant and Child Health Collaborative (MICHC) Resource Guide if anyone would like one.
MICHC completed a community health assessment, she will send to Hillary to send out to the
Membership. She also shared that it is time to test your house for Radon , you can get a free kit from
Environment Health.
Michelle (Catholic Charities) - She shared that she was just trained in the Community of Caring
parenting program. They are now authorized to provide the Common Sense program and the Common

Sense for Toddlers and Preschoolers program. They will have classes in Dansville and Geneseo in the
first part of December. She shared that they would like to do community based groups anywhere that
they are wanted/needed, that has a good/inexpensive space to hold them. They can offer 123 Magic,
Scream Free, Common Sense, and the Nurturing Program (research based). They can also provide
classes in home, 1 on 1, and in home family therapy services for the SPOA Committee. They also
conduct groups in the jails (currently with just female inmates, but want to offer to males - need
funding).
Tom (Health & Wellness) - Nothing new really, they are looking for NYConnects 3x4 slips that they
used to pass out.
Robin (Venture Forth) - The Alzheimer's Association is doing educational training in two sites,
Geneseo and Dansville. They need more attendance if they are going to keep this service here. There
will be a group on 12/08 from 2-3:30PM at the Wadsworth Library on grief and a group on 12/15 from
6:30-8PM at the Mt. Morris Library on dementia and driving. Please share this information with
anyone you think may be interested.
Sue (Office for the Aging) - They are currently conducting an educational campaign on drugged
driving in the elderly population. They have a lot of pamphlets if anyone needs some. They are
currently involved in three partnerships with SUNY Geneseo. They have students that are willing to
be trained to help with income tax assistance for older adults. She will keep the group updated. They
are also working with them on a Martin Luther King Day, day of service. 20-30 seniors participate
with the group. They are also involved in a three year vista project. The first year is primarily the
schools, but they are working as a partner. They are putting together a respite program, about 30
students interested in being trained (going to pilot in February/March). Medicare open enrollment is
right now, so they are very busy. She shared that NYConnects has a new branding program from the
state. Everything is new, so they will be distributing new materials very soon. She will be making
cards like last time. Everything has to be approved through the state now. She shared that the
Livingston County NYConnects site will be phased out, the state wants everyone to use the state site.
Derrik will update the GVHP site to re-route to the new site once it is up and running smoothly. Sue
will let Derrik know when to connect to the new site.
Mental Health - They are currently conducting a training on screening and brief intervention for
substance abuse, when to refer, etc. The initiative is being pushed out to providers as well, trying to
connect Mental Health staff with CASA to become more familiar with them. CASA has domestic
violence classes on the weekends from 9:30-11:30AM. These classes meet the requirement for those
that are mandated. The classes are funded by whoever mandated them or there is a flat fee, it is an 810 week course.
Jenn (Dept. of Health) - The Dept. of Health will no longer be working to service the jail in the
capacity of nurses. Their contract will end at the beginning of the year. The jail will be hiring their
own nurses. Forensic health services will continue through the Dept. of Health. She shared that they
recently updated their software for hospice. She also shared that HCTC was able to get a social host
law passed in the municipality of Geneseo. They are investigating if this law could be passed in
additional areas or possibly county wide. Two social host arrests were made around Halloween.
Compliance Checks were done in the county as well, all passed. The Dept. of Health has also been
very busy with assisting Seneca County with the recent Hep A outbreak.
Jackie (Dept. of Health) - She shared that they have been busy helping Seneca County with their
emergency (Hep A outbreak). They have an MOU through S2AY to assist them. They sent 7 nurses
for 7 days to help with clinics. They vaccinated over 2,800 people. She shared that she is also the

Accreditation Coordinator for the Department, they are going on their third year. They will go through
the re-accreditation process in 2018.
Ruth (Parish Outreach Center) - She shared that in addition to clinics, they are getting a lot of calls for
medications. They are seeing a lot of people in the "donut hole" or pay down is so high they can't
afford them. She shared that through would like some updated brochures for GVHP. HSD will work
on updating the brochures and send them out to the group.
Kevin (Roswell) - The Great American Smoke Out is today. He presented to the Livingston County
Association of Village Boards on the impact of tobacco use and marketing on communities. He shared
that smoking rates have dropped, but e-cig use is on the rise, especially among youth. There has been
some interest in creating policies around point of sale, in Avon. Avon drafted a resolution to protect
kids from tobacco marketing, they contacted him to get more information. They are doing great work.
If anyone knows people on local boards and have concerns about tobacco marketing, just let him know.
He is hoping to build a coalition around this and gain more support. He is also trying to get in the door
with SUNY Geneseo, he spoke to one of the Deans. The American Cancer Association (ACA) put out
a report of colleges in NYS, unfortunately GCC and Geneseo received a D on their report due to their
no smoking policies not being as comprehensive as others. Students at SUNY Geneseo raised
$178,000 for the ACA, so they are very supportive, their policies just need to be revised. He shared
that their seems to be an obstacle with employees that are there, which is a hard hurtle to get over. If
you have any contacts, please let Kevin know. She also shared that the US Dept. of Housing and
Urban Development sent out a recent proposal, that will eventually mandate that all Public Housing
Authorities are smoke free. Jenn shared that smoke free housing is included in the Prevention Agenda
Refresh Chart from the state.
Amy (Noyes) - The hospital held a ground breaking for the new cancer center on 10/28. It is a
partnership of Noyes Health, the Wilmont Cancer Center, and Jones in Wellsville. In two years, the
county will have a comprehensive cancer center. She also shared that in Dansville, the building that
used to be GCC, has been acquired by the hospital and by next spring will house their Mental Health
Services. This will provide a lot more space, which is much needed. She also shared that there was a
press conference sharing that Noyes will become an affiliate of the URMC system. This affiliation
will bring a great amount of savings through purchasing power. This will become effective some time
at the beginning of the year.
Nicole (Catholic Charities) - Christmas is coming, they are taking applications now for Mt. Morris
school district, but have information for all areas, Tammy can direct anyone that calls. Their Annual
Christmas Gala will be held in December, they are officially sold out.
Patty (Noyes) - There will be a Look Good Feel Better program on 11/30 from 10-12pm, this is a
partnership with the ACA. This is the third one to be held this year. The first session had 1 person and
the second session had three people, so growing slowly. There will be a stroke presentation on 12/03
at the hospital. She shared that the Noyes First steps case manager is leaving. If you know anyone that
may be interested in this part time position, please let Patty know. The Chronic Disease Self
Management Program (CDSMP) will continue expand in 2016. There will be four Living Healthy
classes and two Living Healthy with Diabetes classes held yearly. They are looking to bring Living
Healthy with Chronic Pain next year as well. She shared that they are also expanding education
opportunities for professionals working with those that have Alzheimer's. She will keep the group
updated, they just received a grant.
Diane (Social Services) - HEAP opened on Monday, you can apply online or can come in to DSS. She
shared that the Housing Task Force will be doing a PIT count (to get an estimate on the homeless

population in the county) on January 27-28th. December 1st they will be meeting at the Government
Center at 6:30pm to discuss how everyone can help with this. The 2014 count was 48 adults, but they
do not think that this is an accurate number. The group would like to identify ways to get a more
accurate count. Please come to meeting to share your ideas. She also shared that it is Adoption
Awareness Month. At the end of 2014, the Preventing Sex Trafficking and Strengthening Families act
was passed. Many of us work with families and may be involved with this. Diane will keep the group
updated.
Other Topics: Lisa shared that we have worked with SUNY Geneseo students in the past to help with
various initiatives and events (through the Community Health Alliance). This group now has a new
liaison. Lisa met with him last week and talked about projects that they might be able to help with. If
anyone needs help, please let Lisa know and she can connect you.
Next Meeting: The General Membership will be continuing same calendar as last year. The next
meeting will be held on February 18th at 2:00pm. HSD will send out a schedule and reserve the
room. Dates/times/locations will also be posted on website.
