Ordering Guidelines:
To ensure efficient and accurate service, please email this form and your greeting card recipient list to
[email protected]. The recipient list should be in a Microsoft Excel spreadsheet. Please be sure
to include each recipient’s full name and mailing address (street name, city, state, and zip code).

We respectfully ask that you make a minimum gift of $25 per greeting card.
