Now in its sixth year of operation, Shade Tree Clinic has
evolved into much more than a free clinic. The tireless
energy and enthusiasm of our volunteers has allowed us
to expand our patient care mission. Last year alone, 558
medical students, physicians, and community members donated
over 15,000 hours of service to the clinic.

Over the past several years, our patients’ needs
have grown more complex. Many have chronic
conditions that require long-term care, and we
depend on your generous support to continue to
meet our patients’ increasingly complex needs.
While our mission remains the same, the ways in which
we strive to meet our patients’ needs and enhance our
medical students’ educational experiences have evolved.
Throughout the past year, we have focused on three
vital areas of growth:
(1) Extending patient care
(2) Enhancing education
(3) Engaging the community
We have expanded the scope of our Patient Health Education program to include
all 202 STC patients suffering from chronic illnesses including cardiovascular
disease, diabetes, and respiratory disease. Redesigned with a multi-disciplinary
mission, our PHE program places first and second year medical students in a
case-manager role. By working alongside social workers and dietetic interns,
our PHEs are able to support lifestyle change and health improvement for our
sickest patients.
Last year was the inaugural year of our longitudinal primary care elective for
fourth-year medical students, and it was tremendously successful. The elective
has facilitated an enhanced continuity-of-care model and re-energized our preclinical teaching effort.
We have also continued reaching out into the community through health fairs,
preventive health screenings, and health literacy and education classes. We
have begun offering our most popular classes in Spanish as well as created new
programs focused on encouraging safe and healthy lifestyle choices in children.
In 2012, we anticipate a move to a larger facility one mile from our current
Grace Street location. We are committed to serving all of our current patients,
and we look forward to utilizing the additional space to continue enhancing our
clinical and educational services. We thank you for your generous contributions,
which make Nashville a brighter and healthier place for so many people.

FROM THE MEDICAL DIRECTORS
With each year of operation, Shade Tree has grown and
evolved as an organization. Its patients have become more
complicated, the needs of the community have become more
noticeable, and more people and community organizations
look to Shade Tree as a resource.
The clinic is small, crowded, and hectic. Patients fill a
small waiting area while 20+ students and faculty crowd
into an equally tight treatment area. It is common for
patients to drop in with acute problems.
One Saturday, a patient dropped in with an acute problem
that challenged the clinic’s providers and demonstrated
the extraordinary capabilities of our students. The patient was well known
to the clinic when he noted the onset of chest pain. He insisted on coming
to Shade Tree, not an emergency room. He said that he knew the clinic and
trusted they would take good care of him. He presented to the registration
window and the students immediately recognized that he was acutely ill. They
put him into an examination room, took a brief history, performed a limited
examination and ran electrocardiogram. Within minutes, they determined
that he was having an acute myocardial infarction. With this assessment,
they instituted appropriate protocols, aspirin, nitroglycerin, and a call
for paramedics. They notified the Vanderbilt ED which activated the cardiac
catheterization lab. Upon arrival he was found to have a coronary stenosis,
which was successfully dilated and stented. All of this was done with
remarkable efficiency and the patient did very well.
Students and faculty visited the patient the next day and were pleased to
see him feeling well, sitting in a chair and joking. For the next week,
the students were frequently greeted and congratulated for their work,
professionalism, and skill. As directors, we also felt proud of how our
organization had functioned under challenging circumstances.
The students caring for this patient were part of a system, which worked
well that Saturday afternoon. They were not working in a state-of-the-art
structure, but had the knowledge and motivation to make a difference. They
had created a system, not just a clinic. We believe that their time at Shade
Tree is a very valuable part of making them better physicians.

Helping the Knights Manage Diabetes
Jimmy Knight knew exactly how hard it was to raise
a family. When he was very young, his family moved
to Nashville after his father’s death, leaving him to
support them on his own. Eventually he met and
married Diane, and the pair made a comfortable life
in Nashville for over 40 years.
But after Jimmy Knight lost TennCare insurance and developed a staph infection on his leg, they had no place to turn. When they sought healthcare
services in 2005, they were referred to Shade Tree.
“I thought, ‘Shade Tree? What is that?’” said Jimmy Knight. “But now I

The Knights joined the Diabetes Alliance, a program that
helps diabetic patients understand their medicines, meet
people with similar conditions and change their lifestyle for better health
in the long run.
What truly stood out to the Knights at Shade Tree was dealing with
students. At Shade Tree, students are intimately involved with patient care.
They interact with patients; they discuss diagnoses and treatment with experienced physicians; and they prescribe vital medications.
“The most important thing is having someone listen and work with you
to help solve problems,” said Jimmy Knight. “We’ve learned a lot. All the
doctors have been really good, all the time. I can’t say anything bad about
it. I’m proud of the students.”

Turning a Life Around
Sometimes it takes a while for a patient to find
Shade Tree. For Patricia Parrott, it was more than
worth the wait.
After moving to Nashville from Greenville, Miss., to live with her adult son,
she got by with inadequate health care for almost three years. Her medical
conditions began to spiral out of control.

The Patient Health Education (PHE) program is a boon for patients like
Parrott who have chronic illnesses. PHEs make sure patients are eating
healthy foods, regularly checking blood sugar and taking medicine as prescribed. A pharmacy stocked with free medicine and advice from students
empowers patients like Parrott to manage hypertension and diabetes.
Parrott says her family and friends have noticed how much she enjoys
coming to Shade Tree, how much better she appears mentally, and physically, and how her zest for life has increased.
“The students are really interested in the patients and are very sincere,”
she said. “They’re willing to listen to my medical problems and my personal
life. As long as they keep letting me come, I’ll come!”

This year, we continued our training program for all firstyear medical student volunteers. The program touched on
taking vital signs, giving shots, drawing blood, interviewing patients, writing clinic notes and discussing sensitive
topics. All of these topics are covered in a manual given to
each first-year student.
Of course, all of these skills can’t be covered in single halfday session; the purpose is to introduce the students to
the clinical skills they will need to succeed, and to create a
foundation for learning from our experienced third and
fourth year students at Shade Tree.
We also founded a fourth-year program that turns the
Shade Tree experience into a VMS elective credit. Participating students volunteered at 20 clinic sessions and
attended six lectures on teaching methods, social work
resources, quality improvement techniques and evaluation
of programming.
At least one CCL attends each clinic session to
lead learning sessions with students and instruct
new volunteers. CCLs also follow some of our
more complicated patients, and they offer feedback on to how to improve clinic flow, patient care
and student education. This kind of diversity from
many different fields helps when we must refer
patients to specialists or obtain second opinions.

In 2008, STC introduced the Patient
Health Education (PHE) program, a comprehensive case-management program
specifically designed to improve health
literacy and disease management in our
diabetic population. The PHE program,
sponsored largely by a grant from the
Baptist Healing Trust, trained first and
second-year medical students to coach
patients on disease management, has
resulted in marked improvement in
patient outcomes. Over the past year,
74% of diabetics working with a case

discussions. Students will learn basic lab value
interpretation, preventive medicine guidelines,
and how to approach tough conversations with
patients, among other topics.
Simultaneously, we strive to stimulate discussion
between STC service-learners and the greater
VUMC community about the practice of compassionate community medicine and the socioeconomic barriers to maintaining health in the
resource-limited setting of uninsured healthcare delivery. At the Lefkowitz Hour, Shade
Tree’s Clinical Case Conference, students
interact with faculty, staff, and the Vanderbilt
community as they discuss complex cases from
our clinic. Students bring their knowledge of
social work resources, community partnerships
and patient histories to the table, while faculty
members facilitate discussion on improving
clinical medicine.
Finally, Shade Tree inspired the creation of The
Advocate, an informational booklet containing generic drug lists, social work resources
and community agencies. With this resource,
we have enabled our students and providers to
deliver more comprehensive care to our uninsured population.

the PHE program has matured, we have
learned that out-of-clinic follow-up and inclinic care coordination are the most important contributions of PHEs to patient
care; fundamentally, PHEs are patient advocates who help our providers facilitate
coordinated, comprehensive medical care
while simultaneously motivating patients
and helping them navigate the complex
medical landscape. The 1:1 relationship
between PHEs and their patients has
enabled identification and resolution of
unmet medical and social needs.
The patient advocacy that has emerged
from the PHE-patient partnership has led
us to develop a complementary social
work case-management program. This
program pairs medical student social
workers, trained in social services and
public resource management, with
patients who need social service support.
Fourth-year elective students, PHEs, and
social work case managers work together
to ensure that all STC patients receive
comprehensive care.

Out
◼ Shade Tree’s third major focus continues to
be on community outreach programs throughout the greater Nashville area. These programs
began as an attempt to reach beyond our
patient base and improve the health conditions
of homeless and low-income individuals in the
greater Nashville area. Shade Tree has offered weekly vaccine clinics at
community centers and homeless shelters in the
Nashville area, where medical and graduate nursing students collaborate to offer Influenza, TDaP
and Pneumovax vaccinations free-of- charge.
As of the end of 2011, Shade Tree has provided
nearly 4,000 vaccinations to the homeless.
Health Fairs and Screenings
The Shade Tree Outreach Program offers blood
glucose and hypertension screening exams at
many community events. The Outreach team
also organizes an annual community fair for
residents in underserved neighborhoods. More
than 200 people attended the 2010 fair, where
numerous student organizations collaborated to
provide vaccines, social work information, and
screenings for body mass index, hypertension,
diabetes, head/neck cancer, hearing and vision.
Health and Nutrition Education
Shade Tree continues to partner with the Vanderbilt Educational Garden Initiative (VEGI), which
runs a community garden in the McFerrin Park
neighborhood that Shade Tree serves.
Vaccine Clinics
There are an estimated 2,200 homeless living
in Nashville, all of whom are at an increased
risk for contracting communicable diseases.
Additionally, once a homeless person gets sick,
it takes significantly longer to get better due
to unstable and substandard living conditions.
Research has shown that those who lack consistent access to medical care are less likely to
get vaccinated against common illnesses. They
are also more likely to delay seeing a healthcare
provider for infections and other acute illnesses.
As a result, they are more likely to suffer serious
complications or even die from preventable and
treatable illnesses.
For the past three years, thanks to the generous
support of The Boulevard Bolt Foundation,
The Shade Tree Resources in Diabetes Education (STRIDE) and Shade Tree Trim weight
management classes are now entering their third
year of existence. STRIDE is an eight-week class/
support group that teaches those with and at-risk
for diabetes about the disease and how to manage it in a fun, friendly atmosphere. Both classes,
now offered in both English and Spanish, incorporate nutrition education and exercise lessons
into each weekly class session.
Established in partnership with Caldwell Elementary, a local school in the McFerrin Park
area, our newest outreach program (Swim Safely
East Nashville!) fosters healthy lifestyles by offering swimming lessons for grade-school children.
This medical-student-run program will educate
children on fundamentals of water safety while
providing after-school education on healthy
nutrition and exercise in hopes of tackling increased risks of pediatric drowning and obesity
in the East Nashville community.
