BlueRewards, the wellness incentive program provided by ICUBA, is powered by Rally
effective April 1st. Through the new BlueRewards powered by Rally you will no longer
receive POINTS as wellness incentives – now you will receive DOLLARS! The new
wellness incentives will be redeemable for gift cards to online retailers like Sears,
Target, NFL Shop, Fandango and JC Penny. Come out to one of the sessions to learn
more about the program and the great enhancements!
Steps for registering for My Health Toolkit is as followed: •
Go to www.MyHealthToolkitFL.com and select register now.
•
Enter the number from your new medical ID card and date of birth. If
you don’t have your membership card, enter the policy holder’s SSN.
•
Choose a username and password.
•
Enter your email address and choose to go paperless.
How to access Rally for my wellness incentive program?: • Log in to My Health Toolkit® > Select Wellness > then Rally.
• Upon logging in you’ll need to set up your personal profile and picture
and pick an avatar of your choice.
• Once logged into Rally, employees are immediately prompted to
complete the Rally Survey.
