Your transition to the Legacy Employee Savings 403(b) Plan
Welcome to the team!
As part of the transition to Legacy Health, your current Silverton Hospital “Plan Your Future”
Plan balance will transfer to Legacy’s Employee Savings 403(b) Plan on January 1, 2017.
During the transfer, your current investments will automatically be mapped to similar options in
Legacy’s plans.
What to expect during the transfer
During the process of transferring your account, you will be unable to direct or diversify assets
credited to your plan account, obtain loans from the plan or obtain distributions from the plan.
This period, during which you will be unable to exercise these rights otherwise available under
the plan, is called a “blackout period.”
The blackout period for the plan will begin at 1:00 p.m. PT on December 27, 2016 and end
on January 10, 2017 or earlier if possible.
We encourage you to consider carefully how this blackout period may affect your retirement
planning, as well as your overall financial plan. It is very important that you review and consider
the appropriateness of your current investments in light of your inability to direct or diversify
those investments during the blackout period.
As part of the change to your retirement program, all assets in your plan will be automatically
directed to the new investment options chosen by Legacy Health (as shown on the enclosed
mapping notice). These options were chosen because their investment approach and objectives
are similar to those of your current investment options.
When the blackout period ends, you will be able to login to your new Legacy Health account,
review it and make certain changes. You will need to designate your beneficiaries online as
your current beneficiaries will not transfer from the Silverton Hospital “Plan Your Future”
Plan.
What to expect after the transition
You may change your investments at any time after the transition. You’ll also benefit from the
convenience of online enrollment and contribution changes instead of filling out paper forms.
Future employer matching contributions will be made to Legacy’s Matched Savings 401(a) Plan.
Similar to Silverton’s employer match, Legacy’s match is dependent on years of service and a
voluntary employee contribution to the Legacy Employee Savings 403(b) plan. To receive the
maximum amount under Legacy benefits, you must contribute 5% of your salary to the 403(b)
plan.
In addition to your 403(b) plan and employer match, you’ll also have an Employer Contribution
Account. Each year, Legacy will make a contribution to your Employer Contribution Account
based on your years of service. For calculation of the 401(a) match and Legacy’s contribution to
the Employer Contribution Account, your years of service at Silverton will count toward your
years of service at Legacy. Although the fund line up is changing, you’ll still have access to a
range of mutual fund and target-date model options, but now with significantly lower expense
ratios.
Your retirement plan provider is staying the same
Both your Silverton and your Legacy retirement benefits are administered by Lincoln Financial
Group. If you’re already registered on LincolnFinancial.com, your user name and password
won’t change. In addition, the Employee Savings 403(b) Plan, the Matched Savings 401(a)
Plan, and the Employer Contribution Account will appear on the same quarterly statement from
Lincoln Financial. This will make it easier to manage your retirement planning and to see if your
savings are on track.
You choose how your contributions are invested, and you may choose different investments for
each plan. You still have access to mutual funds and target-date portfolio options, and you also
benefit from the significantly lower expense ratios in the Legacy Health investment lineup.

Note: For employees represented by the ONA, the Hospital and the ONA have agreed that all ONA-represented
nurses will transition to the Legacy Retirement Plan and will be covered by the same terms of the retirement plans as
everyone else, unless there is a subsequent agreement between the parties that changes that status. So although
bargaining is ongoing, ONA-represented nurse’s retirement will transition just like everyone else.

HEALTH 403(B) PLAN and the plan’s designated investment alternatives under the Lincoln Alliance
program, including fee and expense information, to help you compare investment options in accordance
with Department of Labor (DOL) Regulation section 2550.404a-5 (“404(a) participant fee disclosure”).
Information presented throughout this entire document is specific to the plan’s designated investment
alternatives only under the Lincoln Alliance program. Please contact your retirement plan administrator for

General plan information
To direct your designated investment alternatives for the plan:
For initial enrollment, you may enroll by completing the materials and returning them as indicated on the
documents provided during the enrollment process.

You may change your elections at any time; transfers are effective the same business day provided the
markets are open and instructions are received before 4:00 p.m. Eastern Time.
The Lincoln Alliance program may restrict the number of transfers you may make among designated
investment alternatives within the product in a given time period. An investment option may apply a
redemption fee or restrict certain transfers. However, transactions associated with market timing – such

as frequent, large, or short-term transfers among investment options – can affect the underlying funds
and their investments. Lincoln therefore reviews the number of transfers that a participant makes within
given periods of time to determine if any transfer attempts to capitalize upon short-term movements in the
equity markets (Market Timing Policy). If so, the participant's transfer activity will be subject to further
scrutiny. Potential market timing or frequent trading may result in future trading restrictions, up to and
including temporary (or permanent) revocation of telephone exchange privileges.
Voting and tender rights associated with mutual fund shares offered under the plan are exercised by the
plan sponsor, if applicable.
The designated investment alternatives available in the Lincoln Alliance program are those listed in the
comparative charts. Please contact your retirement plan administrator for information regarding
designated investment alternatives offered outside of the Lincoln Alliance program, if any (e.g., employer
securities).
In addition to the designated investment alternatives available under your plan, you may invest in a selfdirected brokerage account (SDBA) through TD Ameritrade. Subject to plan restrictions, investing in a
SDBA may provide you access to a broad range of investments that fit your investment goals such as
individual stocks, bonds, CDs, ETFs, and more than 13,000 mutual funds, including more than 2,100 noload, no-transaction-fee mutual funds. Please note that all 403(b) plans may only be invested in mutual
funds.
It is important to note that a SDBA is not a designated investment alternative under the plan. The plan
sponsor is not responsible for and does not review and evaluate each investment option made available
through a SDBA to ensure each investment option is a prudent selection to be made available to plan
participants. Therefore, before you invest in an investment fund through a SDBA, you should read the
fund's prospectus and carefully consider the particular fund's investment objective, risks, charges, and
expenses.
Investments made through a SDBA are subject to commission fees, service fees, transfer fees, and may
include quarterly account fees. The TD Ameritrade fee schedule provided at the end of this document is
delivered to you upon enrolling in the service and is posted on their participant website –
www.tdameritraderetirement.com.
Generally, there is no minimum balance needed to open a SDBA, but certain investments may require a
minimum initial investment. In addition, investments in a SDBA may be subject to trading restrictions and

Plan fees and expenses
This section provides an explanation of fees and expenses for general plan administrative services, if
any, that may be charged to or deducted from all individual accounts and are not reflected in the total
annual operating expenses of any designated investment alternative. For example, the plan may incur
general administrative expenses each year to cover services related to the operation of the plan (e.g.,
legal, accounting, recordkeeping, trustee fees, and Registered Investment Advisor fees). Where
applicable, the plan may charge a portion of these expenses to participant accounts if the expenses are
not paid by the plan sponsor or from revenue sharing payments the plan receives from plan investment
options. These expenses will appear on your quarterly retirement statement, if applicable.
Additionally, an explanation of any fees and expenses that may be charged to or deducted from the
individual account of a specific participant based on the actions taken by that person are described below.
The dollar amount of fees and expenses that are actually charged to a participant’s account during the
preceding quarter will be reflected on the participant's quarterly retirement statement.
General administrative expenses
Retirement plans have expenses associated with them. Here is a list of administrative fees associated
with this plan.

receipt and processing of deposits, processing of withdrawals,

fee is not paid by the plan sponsor or from revenue sharing

The plan applies fees to individual participant accounts. These charges apply only to participants who use

General plan information
To direct your designated investment alternatives for the plan:
For initial enrollment, you may enroll by completing the materials and returning them as indicated on the
documents provided during the enrollment process.

You may change your elections at any time; transfers are effective the same business day provided the
markets are open and instructions are received before 4:00 p.m. Eastern Time.
The Lincoln Alliance program may restrict the number of transfers you may make among designated
investment alternatives within the product in a given time period. An investment option may apply a
redemption fee or restrict certain transfers. However, transactions associated with market timing – such
as frequent, large, or short-term transfers among investment options – can affect the underlying funds
and their investments. Lincoln therefore reviews the number of transfers that a participant makes within
given periods of time to determine if any transfer attempts to capitalize upon short-term movements in the

equity markets (Market Timing Policy). If so, the participant's transfer activity will be subject to further
scrutiny. Potential market timing or frequent trading may result in future trading restrictions, up to and
including temporary (or permanent) revocation of telephone exchange privileges.
Voting and tender rights associated with mutual fund shares offered under the plan are exercised by the
plan sponsor, if applicable.
The designated investment alternatives available in the Lincoln Alliance program are those listed in the
comparative charts. Please contact your retirement plan administrator for information regarding
designated investment alternatives offered outside of the Lincoln Alliance program, if any (e.g., employer
securities).
In addition to the designated investment alternatives available under your plan, you may invest in a selfdirected brokerage account (SDBA) through TD Ameritrade. Subject to plan restrictions, investing in a
SDBA may provide you access to a broad range of investments that fit your investment goals such as
individual stocks, bonds, CDs, ETFs, and more than 13,000 mutual funds, including more than 2,100 noload, no-transaction-fee mutual funds. Please note that all 403(b) plans may only be invested in mutual
funds.
It is important to note that a SDBA is not a designated investment alternative under the plan. The plan
sponsor is not responsible for and does not review and evaluate each investment option made available
through a SDBA to ensure each investment option is a prudent selection to be made available to plan
participants. Therefore, before you invest in an investment fund through a SDBA, you should read the
fund's prospectus and carefully consider the particular fund's investment objective, risks, charges, and
expenses.
Investments made through a SDBA are subject to commission fees, service fees, transfer fees, and may
include quarterly account fees. The TD Ameritrade fee schedule provided at the end of this document is
delivered to you upon enrolling in the service and is posted on their participant website –
www.tdameritraderetirement.com.
Generally, there is no minimum balance needed to open a SDBA, but certain investments may require a
minimum initial investment. In addition, investments in a SDBA may be subject to trading restrictions and

Plan fees and expenses
This section provides an explanation of fees and expenses for general plan administrative services, if
any, that may be charged to or deducted from all individual accounts and are not reflected in the total
annual operating expenses of any designated investment alternative. For example, the plan may incur
general administrative expenses each year to cover services related to the operation of the plan (e.g.,
legal, accounting, recordkeeping, trustee fees, and Registered Investment Advisor fees). Where
applicable, the plan may charge a portion of these expenses to participant accounts if the expenses are
not paid by the plan sponsor or from revenue sharing payments the plan receives from plan investment
options. These expenses will appear on your quarterly retirement statement, if applicable.
Additionally, an explanation of any fees and expenses that may be charged to or deducted from the
individual account of a specific participant based on the actions taken by that person are described below.
The dollar amount of fees and expenses that are actually charged to a participant’s account during the
preceding quarter will be reflected on the participant's quarterly retirement statement.

the plan. These expenses may include, but are not limited to the
receipt and processing of deposits, processing of withdrawals,
call center operations, website development and maintenance,

deducted from your account on a pro rata basis to the extent the 0.0150% / quarterly
fee is not paid by the plan sponsor or from revenue sharing
payments the plan receives from plan investment options. If
revenue sharing payments are received, only the portion of the

Legacy Health maintains the Legacy Health 403(b) Plan and Legacy Health Matched Savings 401(a) Plan to help you attain
financial security during your retirement years. As a participant, you decide how your retirement plan dollars will be invested in
the designated investment alternatives available in the plan. If you do not make an investment election, your contributions will be
invested in the Qualified Default Investment Alternative (QDIA).

When the default fund will be used:
The plan is a participant-directed plan, giving you the right to direct the investment of assets in your plan account among the
designated investment alternatives available in the plan. In the absence of an investment election, your future contributions will
be invested in the plan’s QDIA under any of the following circumstances:

You have made a salary reduction election without an investment election.
•
An employer contribution has been made on your behalf but you have not provided an investment election.
•
The plan contains an automatic enrollment feature and you did not make an investment election during the notice period. 3%
of your eligible compensation will be contributed to the plan as an elective contribution. You have the right to elect not to have
such automatic contributions made to the plan on your behalf and the right to elect to have such automatic contributions
made at a different percentage. Please contact the Plan Administrator for more information.
You have made investment elections that do not equal 100% or you have elected an investment that is no longer available in
the plan.
If the assets in your plan account are invested in the plan’s QDIA, you can direct the investment of those assets to any other

Basic Financial Statement
Benefits under the plan are provided by insurance contracts and a trust fund. Plan expenses were $41,622,298.

This notice provides important information regarding the 403(b) plan in which you are eligible to participate (the
Plan).
You may want to take this opportunity to either (1) begin making pre-tax 403(b) elective deferral contributions or (2)
review your current elections and decide if you want to make changes. Before making any initial elections or
changes, be sure to consult the written plan and any other materials provided to you that explain the terms of the
plan.
When can I enroll?
You are eligible to enroll immediately upon your date of hire.
You may choose your initial elective deferral rate by completing the appropriate application and any other necessary
forms at the time of your initial enrollment.
Follow your plan’s normal process to obtain either a Salary Reduction Agreement or an enrollment form so that you
may choose your elective deferral rate. This may be achieved by visiting LincolnFinancial.com, speaking with a
customer service representative, or requesting it through your plan administrator.
Can I change or stop my elective deferral contributions?
You may change your elective deferral contributions once per pay period during the plan year. You are permitted to
revoke your election at any time during the plan year.
When are my elective deferral contributions effective?
After completing the enrollment requirements, your elective deferral contributions will begin on the next pay period
or as soon as administratively possible.
What is the maximum amount that I can contribute?
The Internal Revenue Code limits the annual contributions you can make to a 403(b) plan and the limits are

* The 15 years of service catch-up contribution applies before the age 50 catch-up contribution and is based on a
formula that takes into account all past contributions to the plan and the employee's total years of service to the
employer. The maximum allowable for the 15 years of service catch-up is $3,000 per year up to a $15,000 lifetime
benefit, but an employee's actual catch-up may be lower than this maximum.
Will my employer make additional contributions?
In addition to your pre-tax 403(b) elective deferral contributions, the plan may allow for additional employer
