POSITION DESCRIPTION POSITION DETAILS: Position Title: Mental Health Recovery Worker– Mantle Apartments Employment Status: Part Time/Full Time Classification: Social, Community, Home Care & Disability Services Industry Award, 2010 Level 4 Reports to: Mental Health Team leader - Mantle apartments Reporting to this position: Nil Position Summary: Under direction from the Team Leader implement a mental health Recovery Orientated Practice Model supporting 14 clients in the Mantle Apartments. Mantle Apartments are a new purpose-built accommodation model for 14 young adults living with mental illness who are currently cared for by their ageing parents. This holistic and innovative accommodation has utilised a recovery orientated practice co-design model from inception, building design and build, through to transition and service model development. With 24-hour wellbeing support on-site and the provision of tailored individually designed and delivered support, Mantle apartments offers an opportunity to be part of something truly innovative. Ensure individual and team practice is in accordance with the requirements of relevant legislation including the Human Services Quality Framework and the National Mental Health Standards. 3. Work in accordance with psychosocial rehabilitation, social inclusion and recovery orientated practice frameworks. 4. In partnership with clients and key stakeholders, develop, implement and review individual recovery plans through individualised goal planning, progress monitoring and regular reviews which are flexible and responsive to the individual’s needs and achievement of goals. 5. Support clients to arrange and manage outings, meetings, medical appointments, personal care tasks and other daily living tasks including housekeeping, shopping and cooking based on their personally identified needs and recovery goals. 6. Ensure client information is up to date and in accordance with case requirements. As necessary draft documentation and reports for a range of stakeholders. 7. Undertake professional development which includes regular supervision and reflective practice. 8. Participate in client case reviews within and external to Wesley Mission Brisbane to ensure that client service continuity, an individualised approach to services and clear pathways of transition to a more independent life is achieved. 9. Identify and assist in locating resources, services and supports that facilitate community integration including social, educational, vocational and leisure activities. QUALIFICATIONS, KNOWLEDGE AND REQUIREMENTS: Essential: Minimum Certificate IV in Mental Health or Peer Worker or a related area. Current Driver’s Licence and willingness to drive in the course of work. Relevant key personnel and probity checks required by legislation and WMB policy. Passionate about making a positive change in the lives of people managing mental illness. Understanding of the principles of recovery in the mental health setting. Demonstrated knowledge and appreciation of the issues associated with living with a mental illness. Demonstrated experience working with people with complex mental health support needs across the phases of service delivery including assessment, recovery oriented practices, community engagement and evaluation of services delivered. Capacity to work independently and collaboratively within a team environment including being accountable for the effective delivery of client services. Sound communication and interpersonal skills including the ability to engage, negotiate, advocate, collaborate and undertake joint innovative problem solving. Demonstrated effective PC skills including MS Office and related applications packages (including reporting). Highly Desirable: Tertiary qualifications in community, social welfare, health or a related discipline. Understanding of the principles and standards of the Disability Services Act (Qld 2006) and the National Mental Health Standards and how they relate to providing effective and accountable services. Experience in working within a recovery orientated service frameworks for mental health.
