NOTES FOR STUDENTS
1.
You will be able to order your transcript online and receive it either by ordinary mail or by personal collection, UNLESS any of the
situations apply to you:
(a) The transcript needs to be dispatched to another institution to substantiate your application for admission to study in that
institution.
(b) You would like the transcript to be posted to an address outside Hong Kong.
(c) You would like the transcript to be delivered by local registered post.
(d) The transcript is required urgently.

You may order your transcript if your study is not discontinued by the College. The charge for each copy of transcript is HK$50.00
which is non-refundable. For online application, a debit note number will be generated upon submission of application. Payment
could be made by ATM, PPS or internet banking. For form application, please settle the payment at the Information Kiosk on either
campus with the Octopus card. Payment receipt as evidence of payment will be required.

The application procedures for Transcript of Studies are as follows:
(a)
Online application
(i) Please go to: http://www.cpce-polyu.edu.hk/hkcc/its/myhkcc/.
(ii) Follow the guide online.
(iii) You will receive a payment advice in which the debit note number is stipulated, upon submission of your request online. Your
order will be processed only upon receipt of your payment.
(b)
Form application
(i) Complete Sections I to V of this form.
(ii) Settle the payment with one of the methods specified in point 2 above.
(iii) Return your application form and the payment receipt to the College+ in person. If you would like the transcript to be
posted to you by local registered post, or if the transcript is to be sent to another address either within or outside
Hong Kong, your application should be submitted with postage. Please make payment with your Octopus card.
4.
If you would like to have the transcripts sent to overseas addresses, you are required to pay additional air postage at the following rate
(for addresses outside zone 1 and 2, please visit the Hongkong Post website* for updated rates on postage and registered mail:

The Transcript of Studies is sent by registered mail to a local address.
A Transcript of Studies without any enclosure weighs 30g.
For registered mail to overseas, an additional HK$15.50 shall be added.

To avoid postal error, you are requested to type the address(es) of the recipient(s) of the transcript(s).

The processing time for issuing a transcript is normally seven working days. Every effort will be made to ensure that the request is
entertained on a timely manner except where we are in examination or at semester commencement period, during which the processing
time will be extended to 10 working days. For urgent cases, please indicate the date required and attach documentary evidence to your
application. In case if the request could not be acceded to, you will be informed by telephone.
7.
Your transcript(s) will be posted to the mailing address(es) as indicated in your application. The College does not bear any
responsibilities for postal loss due to errors in typing or other reasons. If you wish to collect the transcript in person, you will be
notified of the collection by telephone or via email. Authorised application and/or collection is acceptable and please provide your
authorised representative with a letter of authorisation duly signed by you and a photocopy of your Hong Kong Identity Card. To
protect your privacy, your HKID card copy will be returned to your representative after verification.
8.
For security reason, your transcript will be destroyed if it remains unclaimed for three months from the date of issue as printed on the
transcript.
9.
The information given in this application form will only be used for processing this application.
