Application Instructions
Berlin: Free University Spring 2011-12
You must submit the following two forms to the UCEAP Systemwide Office for your application to Free
University. Detailed instructions on how to complete these forms are in this document. Make sure you
read through the instructions carefully.


1. Application for Admission

Fill this form out on your computer.

Use European date format: Use the day.month.year format. For example, to write January 17, 2012,
you would write “17.01.2012.”

Term: Check “Summer term” and write “2012”

Personal Information: Fill in your biographical information as requested on the form. This is selfexplanatory, however as noted above, use the European date format (day.month.year) for your date
of birth.

Subjects of Study: List which subjects you intend to study while at Freie University (refer to the
listed website to get an idea of subjects offered).

Mailing Address: List your summer or permanent address. Be sure to list the dates you will be at
this address (in European date format).

Former Universities and Degrees: List any former colleges that you have attended, including junior
colleges or community colleges. Include your area of study and the dates that you attended (in
European date format). Also include the information for the UC you are currently attending (you can
leave the ‘Until’ column blank).

German Language Knowledge: Answer questions accordingly. Include any Advanced Placement
tests you have taken and the score that you received.

Print the form once completed.

SIGN and DATE your form! For “Place,” include the place where you are when you sign the form.

Make one photocopy.

Submit the completed form and one photocopy to the UCEAP Systemwide Office by the
deadline.

2. Enrollment Application

Fill this form out on your computer, before printing it.

Use European date format: Use the day.month.year format. For example, to write Janurary 17,
2012, you would write “17.01.2012”

The English translation for each field on this form is listed in small font under the German
instructions.
o Term: Fill in “2011/2012” for your year and circle “Summer-“ for your term
o Exchange Program/Sponsoring Organization: EAP University of California
o Course of Study: Your major or intended area of study while in Berlin
o Name: Last Name, First Name
o Maiden Name: Only fill in this field if you have changed your last name
o Date of Birth: Be sure to put your date of birth in European Format, Day/Month/Year
o Sex: Female- W; Male- M
o Place of Birth: Indicate the city, state, and country in which you were born
o Nationality: Fill in your nationality (e.g. United States of America),
 International License Plate Abbreviation: USA (if you are not a US national then
leave this field blank)
o Mailing Address: enter your mailing address; street name and number
o International License Plate Abbreviation: USA (if you do not live in the US then leave this
field blank)
o Zip Code and City: Fill in the rest of your mailing address; zip code, city, and state
o E-Mail Address: Fill in your primary e-mail address
o High School Graduation: Fill in the year and country in which you graduated high school
o Previous Studies:
 List any former foreign colleges that you attended on previous study abroad programs.
Include the country in which each college was located and the number of months that
you attended (e.g. University of the West Indies, Barbados, 6 months).
 If you have never attended a university in Germany, check ‘nein/no’ and leave the
remaining fields up to the signature blank.
 If you have attended a university in Germany, check ‘ja/yes’ and fill in the remaining
fields.
Once you have completed the form, print it and SIGN and DATE your form (use European date
format)!


Make one photocopy.

Submit the completed form and one photocopy to the UCEAP Systemwide Office by the
deadline.
