The open enrollment period begins Monday, November 2rd through
Friday, December 11th, with changes effective January 1, 2016
 If you are not making any changes to your medical/dental you don’t need to do
anything
 All employees who want to enroll in the voluntary life insurance, short term
disability or long term disability must re-enroll
 All employees who are making changes must complete the appropriate paperwork.
– Make changes to your current medical/dental plan(s)

– Enroll in Flexible Spending Account/Transportation Account
– Enroll in voluntary life insurance, voluntary short term disability, or voluntary

• Eligible participants deferring 0% to 1.99% will be enrolled or increased to 2%
• Eligible participants deferring 2% will be increased to 3%
• Eligible participants deferring 3% will be increased to 4%

• Anyone contributing at or over 5% will continue with their contribution

About your Employer-Paid Life Insurance
Life insurance provides your loved ones with a payment, based on your
coverage, upon your death. It can help your loved ones cover immediate
and future expenses when you can’t be there.
With the plan, you have:
• coverage equal to $25,000
• additional coverage if you suffer serious injury or have a fatal
accident
• Accidental Death and Dismemberment equal to your life

• Pays you 60% a week, up to $1,000 per week for up to 11 weeks
when your disability claim is approved

Transportation Benefit Plan
IRS Rules
• Elections for payroll deductions can be changed on a proactive
basis at the beginning of a month.
• You may: Start – Stop – Increase – Decrease
• Parking/Transit funds do not have to be used within the plan year.
• Unused funds per benefit carry-over from month-to-month and yearto- year.
• An active employee must submit a claims for reimbursement within
180 days (6 months) of the occurrence of the expense.
• A terminated employee has 90 days to submit a claim for
reimbursement that was incurred prior to their termination date.
• Any unused funds that cannot be claimed by a terminated employee
will be forfeit.
• You must complete an Enrollment Form to either participate or to
dis-enroll.
