STUDENT ENROLLMENT AGREEMENT
This Student Enrollment Agreement (which is referred to herein as the “Agreement”), is a
binding agreement between you, the Student, and Smart Horizons Career Online High School
(which is referred to as either the “School” or “Smart Horizons Career Online High School”).
This Agreement is to be read in conjunction with the together with the Student Handbook. The
Agreement becomes biding and effective upon the School’s written acceptance thereof, and
upon compliance with the terms and conditions set forth below.
1.
ADMISSIONS CRITERIA: All students entering Smart Horizons Career Online High
School must be at least 16 years of age prior to the first day of class and have successfully
completed the 8th Grade at an accredited institution. Minors will be required to obtain parental
consent to enrollment and acknowledgement of responsibility.
2.
NON-DISCRIMINATORY CLAUSE: Smart Horizons Career Online High School will
never deny admissions to anyone on the basis of race, color, creed, sex, or national origin.
3.
TUITION: The School offers two different tuition payment options: Option 1:
Full Payment Plan; and Option 2: Automatic Periodic Payment Plan. The details of the separate
options are detailed below.
Option 1: Full Payment Plan. Under this plan, you, the Student, pay the entire tuition
amount upon enrollment. The tuition amount due is $1295. Your enrollment in the
School will not be effective until the School receives full payment of the tuition amount.

Payments are to be made in U.S. dollars. With Option 1 Full Payment Plan, the School will
waive the $100 Administrative Fee.

Option 2: Auto-Payment Plan. Under this plan, you, the Student, pay the tuition in
monthly installments. The tuition amount due is $1395, and there is a $100
Administrative Fee. Your enrollment in the School will not be effective until the School
receives the first payment of $189. Thereafter, you will be charged $67 per month, for
the next 18 months, with the charges being made to an approved debit or credit card.
The first monthly payment is due thirty (30) days after the Enrollment Agreement has
been accepted. Specifically, Option 2 provides:

Payments are to be made in U.S. dollars. With Option 2 Auto-Payment Plan, the School will
charge a $100 Administrative Fee. You are responsible for updating credit or debit card
information should the card expire to avoid any issues.
4.
LATE PAYMENT POLICY: You, the Students, are responsible for your monthly
payment obligations, and these payments must be received in-full by the due date. Depending
your payment cycle, tuition payments are due on the 1st of each month. You are given a fiveday grace period to arrange payment. If you fail to make your monthly payment within five days
of its scheduled due date, you will be sent, via e-mail, a late-payment notification and assessed
a $10 late payment fee. If you fail to make payment ten days after your scheduled payment
date, your account will be suspended and you will be responsible for the monthly payment
installation (as well as all other installations coming due, if any), a $10 late payment fee, and a
$10 reactivation fee prior to being reactivated in the system.
Upon late payment notification, your access to the student portal will be restricted.
5.
INSUFFICIENT FUNDS POLICY: You, the Student, is responsible for your monthly
payment obligations, and these payments must be received in-full by the due date. Nonpayment as a result of insufficient funds is subject to additional fees, as set forth in the late
payment policy above.

6.
SMART HORIZONS CAREER ONLINE HIGH SCHOOL PROGRAM: While the
School’s program is detailed more fully in the Student Handbook (which is incorporated herein),
in general, the Smart Horizons Career Online High School Program, includes:


7.
GRADUATION REQUIREMENTS: In order to graduate from the School and to
receive a diploma, certificate in career major, and high school transcript, you, the Student, must
successfully complete all graduation requirements as specified in the Student Handbook (which
is incorporated herein), pass all assignments with a seventy (70%) percent average, and satisfy
all financial obligations to the school. To be clear, diplomas will not be issued, and transcripts
will not be released, until all fees are collected in full.
8.
WITHDRAWAL POLICY: To cancel your enrollment, please contact the
Enrollment Office at 800.261.6248. Although your enrollment may be cancelled either orally or
in writing, we recommend that you provide a written notice of your request to cancel. All
cancellations are subject to the School’s refund policy.
9.
REFUND POLICY: One of the unique features of the Program is that you are able
to complete it at your own pace. Accordingly, the School’s tuition refund policy is based upon
the duration of your enrollment in the School, regardless of the number of credits completed.
The below schedule sets forth the refunds available to you, depending on when you cancel your
enrollment. Please note that any refund request is subject to a $100 administrative fee for
processing:

For students on the Auto-Payment Plan, your approved debit or credit card will continue
to be charged until the tuition is paid in full in accordance with the above refund policy.
10.
GROUNDS FOR TERMINATION: Your enrollment may be terminated for several
reasons, including, your failure to comply with the School’s rules and policies, failure to comply
with the terms and conditions set forth herein and in the Student Handbook, or the failure to
make scheduled payments when due. The School may terminate your enrollment at any time
for such violations. The School further reserves the right to modify the rules and policies in the
Student Handbook, and any such revisions will be posted online at
www.smarthorizonsonline.org.
11.
ACCREDITATION: You, the Student, understand that Smart Horizons Career
Online High School, within the Smart Horizons Career Online Education District, is currently a
candidate for AdvancED/SACS District Accreditation, with the goal of obtaining accreditation by
January 2011. Accordingly, you, the Student, understand that UNTIL ACCREDITATION IS
OBTAINED, ANY HIGH SCHOOL DIPLOMA MAY NOT QUALIFY FOR ADMISSION INTO A FOURYEAR COLLEGE, COMMUNITY COLLEGE AND/OR CAREER COLLEGE.
12.
CLASS RELEASE SCHEDULE: You, the Student, understand that upon enrollment
in the School, you will first be enrolled in career major elective courses. Each career major
requires completion of eight (8) semester elective courses. Thereafter, the remaining courses
will be made available to you upon a course release schedule. You, the Student, understand
that THERE IS NO GUARANTEE THAT COURSES REQUIRED FOR COMPLETION OF THE SCHOOL
WILL BE AVAILABLE IMMEDIATELY UPON COMPLETION OF THE CAREER MAJOR ELECTIVE
COURSES. SMART HORIZONS CAREER ONLINE HIGH SCHOOL ANTICIPATES RELEASE OF ALL
COURSES BY LATE FALL 2011. IN OTHER WORDS, YOU UNDERSTAND AND ANITICIPATE THAT
YOU MAY NOT BE ABLE TO COMPLETE THE SMART HORIZONS CAREER ONLINE HIGH SCHOOL
UNTIL JANUARY 2012 AT THE EARLIEST. YOU UNDERSTAND AND AGREE THAT THE
COMPLETION DATES ARE NOT FIXED OR WARRANTED AND ARE SUBJECT TO CHANGE, WITH OR
WITHOUT NOTICE TO YOU.
13.
NO GUARANTEE: YOU UNDERSTAND AND AGREE THAT COMPLETION OF THE
SMART HORIZONS CAREER ONLINE HIGH SCHOOL, AND OBTAINING OF A DIPLOMA AND
CAREER MAJOR CERTIFICATE, IS NOT A GUARANTEE OF EMPLOYMENT OR ADMISSION TO A
FOUR-YEAR COLLEGE, COMMUNITY COLLEGE AND/OR A CAREER COLLEGE. SMART HORIZONS
CAREER ONLINE HIGH SCHOOL HAS NOT MADE ANY PROMISE TO YOU, EXPRESS OR IMPLIED,
REGARDING ANY OPPORTUNITIES FOR YOU FOLLOWING COMPLETION OF THE SCHOOL. YOU,
THE STUDENT, UNDERSTAND THAT EACH FOUR-YEAR COLLEGE, COMMUNITY COLLEGE AND/OR
CAREER COLLEGE HAS ITS OWN MINIMUM ADMISSION STANDARDS, AND THE DIPLOMA AND

CAREER MAJOR CERTIFICATE MAY BE ACCEPTED AS MEETING THE MINIMUM ADMISSION
STANDARDS. YOU, THE STUDENT, AGREE TO RELEASE, INDEMNIFY, DEFEND AND HOLD
HARMLESS SMART HORIZONS CAREER ONLINE HIGH SCHOOL, ITS OFFICERS, DIRECTORS,
EMPLOYEES, AND AGENTS FROM ANY SUCH CLAIM.
14.
JURISDICTION FOR DISPUTES: Any suit, action or proceeding arising out of or
relating to, and/or to enforce, this Agreement may be commenced and maintained only in a
Florida state court of competent subject matter jurisdiction in Escambia County, Florida, and
each Party waives objection to such jurisdiction and venue. The Parties expressly, knowingly
and voluntarily waive any right to a trial by jury in any proceeding arising out of or relating to,
and/or to enforce, this Agreement. This Agreement shall be construed and enforced in
accordance with the laws of the State of Florida, without regard to its conflict of law provisions.
Having agreed to the terms and conditions set forth above, the Agreement is duly
executed by the parties on the date last executed below.
