There are many things to think about when you are affected by dementia, be it as the person who
has been diagnosed or as a family member or carer.
The Dementia Information Prescription includes a list of key services, organisations and
contact telephone numbers for Bath and North East Somerset where you may be able to access
information and support. An * highlights where a service offers dementia specific support. Other
things that you may wish to consider are included in a list at the end of the prescription, but as
everyone’s life and dementia experience is unique it does not mean that you will need information
about every subject. We aim to provide you with relevant information when the time is right for you.
We can also provide you with up-dates and an Information Prescription localised to your postcode and

Dementia Adviser/Support Worker – for face to face support and information
Alzheimer’s Society*
Alzheimer’s Society offers information and support for people with
any type of dementia and their families.

Alzheimer’s Society Dementia Support Worker – Katy Berwick*
A Dementia Support Worker provides support to people with dementia
and their carers/families to help them to maintain their independence,
improve their sense of well-being and keeping in control of their
lives. They also assist in identifying needs and access to appropriate
information. This service is provided through one to one contact.

Carers’ Centre
Offer a range of services and a variety of events at different venues.
Most importantly by joining the Carers’ Centre, people can share
their experiences of caring and use it to help others. This support
applies to carers of all ages.

Alzheimer’s Society Day Centres*
Day care enables the carer to have some short term regular respite. The person with dementia
attends a specialist centre for 3 – 5 hours at a time, perhaps one day per week to include
meals and refreshments. Activities are organised either at the centre or as visits out to places
of interest. Often transport can be organised.

The Limes Day Support Service, Alzheimer’s Day Support Service*
A day support service for younger people with dementia (under
70 years). The service provides an opportunity to socialise with
other people with dementia whilst pursuing personal hobbies and
interests. Following assessment the service will also provide you with

Home Support Services offer a personalised one-to one support for
you in your own home.
You will be supported to participate and continue with your daily
life and activities that are important to you. You will be able to make
choices on how your time is spent.
Home Support Services will help you to feel less isolated and help
you to feel independent and in control. Visits will be tailored to your
individual needs.
The services also aim to support those who care for you, by allowing
them time to pursue their own hobbies, and to look after their own

The Peggy Dodd Centre*
Day Centre*
Provides support for older people from Bath and surrounding areas
with Alzheimer’s and other types of dementia. Referral is usually
through a social worker or Community Psychiatric Nurse. It offers, at
very reasonable rates, up to five days per week day care and lunch is
provided. Other services available include chiropody, aromatherapy,
bathing, hairdressing and a home sitting service.

Local support groups for family, friends and carers CONT...
RICE (Research Institute for the Care of Older People)*
RICE hold courses specifically for the carers of individuals with
dementia.
The course consists of 4 sessions which are usually held over four
consecutive Friday afternoons. These run from 1.30 pm – 3.30 pm at
the RICE Centre building at the Royal United Hospital in Bath.
RICE also offer a sitting service so if your relative or friend wants to
come along, members of our staff (nurses and psychologists) can
look after them while you attend the course. We usually organise
some games / activities or just chat and have refreshments.

Community Memory Screening Clinic*
These are clinics run by nurses or psychologists for people over 50
years of age who live in Bath and North East Somerset and who
feel that they have a memory problem. The purpose is to distinguish
between early dementing illness and memory/concentration
problems due to other causes such as stress and bereavement.
These clinics are held in Bath, Radstock, Keynsham, Bradford on
Avon and Trowbridge. Each centre holds a clinic every 2 months.

The Community Dental Service (CDS) provides dental care for
people who are unable to access treatment from a general dental
practitioner (GDP) because of special needs, or disabilities. Home
visits are available where patients are housebound, or where the
disability is such that the individual would find it too difficult to visit a
clinic, but this would be at the discretion of the service.

RICE (Research Institute for the Care of Older People)*
• can give an in-depth assessment of the person’s memory
function and cognition (thinking processes)

for people with severe walking difficulties who travel as drivers or

Care and Repair Home Safety Check and Hospital Discharge
Service. (Falls Prevention Services)
Provide a dedicated service for all hospital staff with a discharge
role, and act as a co-ordinator in the facilitation of safe effective
discharges, by offering a dedicated free Home Safety Check to
those patients who are vulnerable or would benefit from the service.
This service is also available to anyone in the community who is at risk
of falling and feels unsafe, or have any housing conditions which may
be affecting their health and wellbeing.

Offers a specialist support service to people living at home. Once you
have decided that Crossroads Care Wessex is the right homecare
agency for you it will produce a care plan that forms the basis of the
service agreement between you and Crossroads Wessex. This will be
decided jointly and will set out all of the main things that you need help
with, paying particular attention to any specific care needs that you may

Legal and Financial Help – a Lasting Power of Attorney (LPA) enables you to appoint one
or more people who you trust to manage your affairs should this become necessary. The role
of the LPA may be to make decisions about your health and personal welfare that are in your
best interests such as: where to live; the provision of day-to-day care; medical treatment and /
or managing your financial affairs.
You do not have to involve a solicitor to draw up an LPA, more information about the two types
