Patients may access their private health information through our electronic
patient portal. Through this portal you may obtain information such as lab
results, medication and allergy lists. If this is of interest to you, please inform
our front desk receptionist at the time of your visit and you will be provided
with a personal log in.
For your initial office consultation, please bring the following:
▪Medical insurance card and a picture form of identification (drivers license)
▪A list of all medication that you are currently taking
▪Current pharmacy contact information (name, telephone, address)
▪Referring physician and or primary care physician’s contact information
▪All diagnostic studies- MRI, CT scan, or X-rays films, CD’s, and reports
