The Friendship Quads tournament is back in Kenner La, Coconut Beach is the LARGEST outdoor beach facility in the
USA! ALL athletes are encouraged to make new friends/contacts for future doubles partners during this
beach event.

MEDICAL RELEASE FORM
All participants must turn in a signed and completed USA Volleyball Medical Release Form. The form can
be found online at http://www.teamusa.org/USA-Volleyball/Membership/Forms-and-Information.aspx.
The form does not need to be notarized but we recommend that all Junior members participating
in outdoor events carry a notarized copy of this form to all competitions and practices.

REGISTRATION INSTRUCTIONS
Return the completed registration form, all player medical releases, and team entry fee to the address
below. Registration is not complete until all materials have been received. Your registration will be confirmed
via e-mail sent to all team members. One substitution will be allowed at this event, so you do have the option
to register five players per team
All participants must be registered members of USA Volleyball. All participating coaches must be registered
members of USA Volleyball, have a current background screen, and hold a minimum level of IMPACT
coaching certification.
