in the report. The study has determined substantial compliance with applicable

DESCRIPTION OF FINDINGS & CONCLUSIONS
A. Description of Facility
Daycare center is located within the St. Andrews Episcopal Church on Main Street.
Daycare rooms approved for care are 4 rooms located on the second floor. Room #1 is
the infant room, which measures 21x11=231square feet, less 50 square feet, equals a
capacity of 5 infants.
Room #2 is toddler which measures 10x16=160 square feet , less 35 square feet,
equals a capacity of 5 toddlers. Room #3 is preschool which measures 11x12=132
square feet, less 35 square feet, equals a capacity of 4 preschoolers..
Room #4 is 12x17=204 square feet, less 35 square feet, equals a capacity of 6 children.
Total capacity is 20 children. The library will be added after a centralized alarm system
is installed. This area measures 18x25=450 square feet, less 35 square feet, equals a
capacity of 13 children.
There is an uncarpeted area on the first floor, which will be used for an eating area/art
area.
The outside play area exceeds 1200 sqaure feet. and is fenced.
There are 5 sinks and 5 toilets for the daycare children’s use.

B. Program Description
This program runs from Mon. through Fri. 7am until 6pm. Ages range from birth to 12
years of age. There is an ample amount of equipment and toys for the number and
ages of children served.
Program Director is Gerad Heath Meteyer.
C. Rule/Statutory Violations
The two cited violations have been corrected.

RECOMMENDATION
I recommend issuance of a provisional license to this child day care center.
