Teen Camp 2017
Parent Pick Up Preference
I would prefer my child is taken back to the SeaTac Community Center after
teen camp.
I understand that after camp (4:00pm most days) my child is signed out and is no longer in an
organized City of SeaTac program.
I would prefer my child leave directly from the Valley Ridge Community Center.
I understand that staff will be leaving the site at the end of camp (4:00pm most days) and that
my child will either walk home, be picked up, or will be waiting in the park without staff
supervision at that time.
I am signing my child up for after camp extended care at Bow Lake elementary.
I understand that my child needs to be picked up at Bow Lake no later than 6:00pm and that I
will need to bring picture ID with me. Late fee at Bow Lake is $1/minute after 6:00pm and
$2/minute after 6:30pm.

Elevated Sportz
Elevated Sportz requires a Liability Release form signed by participant and parent.
My child has been to Elevated Sportz in Bothell before and I have
completed a Elevated Sportz liability release form.
My Child has not been to Elevated Sportz and I will fill out their liability
release form and return to you as soon as I can.
