Instructions For Making Sample
Follow the instruction sheet and diagrams to make a sample of Celebration Necklace,
Island House (2 color combinations), Parrot Fish, and Yellowtail Fish. Patterns increase in
difficulty from necklace (easiest) to Yellowtail Fish (hardest) and take about 10-15 minutes to
make. More samples will be needed for groups larger than 3 persons.
When To Use This Activity
Beading is a remarkably adaptable activity suitable to persons with severe disability as
well as those with moderate to mild impairments. The simple and repetitive actions of putting a
bead on a string can be done at 3.4; when two ends of a beaded string are tied together, a short
sequence of actions can be introduced with a goal resulting (3.8). Patterns using a few colors and
shapes require attention to striking features, which can be attended to singly (4.2) or in short
groups (4.4.) Working with two strings at once requires attention to the direction of beading
sequence. The projects in this kit are designed to identify and use available cognitive structures
from 3.0-4.8. More difficult patterns can be found in Bead Kit II (CU-246.)
Time to completion is short (about 15-30 minutes for most persons) and groups of up to 4
may be monitored by one therapist. Groups of more than 3 persons will require more samples.

Evaluation Set-Up
Individual
Make samples. Pre-cut elastic or rattail cord. Attach lanyard hook to key ring. Count out
exact number and type of beads for the kit (see instructions.) Place beads, key ring/hook, and
cord or elastic in bin within easy reach. Have available masking tape, scissors, and an extra cord
and key ring for demonstration purposes. Based on known ACLS or anticipated ACL, set-up all
supplies and sample within 24˝ (60 cm) as follows:
3.0 – 4.2 Celebration Necklace
4.2 – 4.4 Island House
4.4 – 4.8 Parrot Fish or Yellowtail Fish
Group
Prepare kits and set-up as for individual assessment. Larger groups will require additional
samples (no more than 2 persons sharing a sample is preferred.)
Therapist’s Instructions (3.0-4.2)
Celebration Necklace
(3.0-3.4): “This is a necklace. You can make one for yourself. Place the beads on the
elastic like this.” (Demonstrate one bead.) “Now you try it.” Observe, re-direct attention, rate
performance on rating criteria.
(3.6-4.2): “This is a necklace. It is made by putting beads on an elastic cord and tying the
ends together. You can keep it or give it away as a gift. All the supplies needed to complete this
necklace are in front of you. In order for me to see how you are doing today, it is important that
you try to make your necklace look as much like the sample as possible.”
Give the following instructions, while demonstrating on an extra elastic cord: “Pick up a
white bead and tie it near the end of the cord with a knot. This will keep the beads from coming
off as you make the necklace.” (Demonstrate and observe performance.) “Find the knot on the
sample. Notice that there is white bead next to the knot: this is where you will start. Pick up a
green bead and put it on the elastic cord. Continue to put beads on the elastic, following the
color pattern on the sample. When you are done, untie the first white bead and make a knot
with both ends.”
Observe behavior, offer assistance, and rate behavior using rating criteria.
Therapist’s Instructions (4.2-4.6)
Island House/Parrot Fish/Yellowtail Fish:
“This is a beaded key ring.” (Optional for dementia: “What does this shape remind you
of?”) “It is (“a red-tile roof island house, the kind you find in the Caribbean Islands” or “a fish
called a Parrot Fish that is found in the Caribbean” or “ a fish called a Yellowtail Fish that is found
in the Caribbean). All the supplies needed to complete this key ring are in front of you. In order

for me to see how you are doing today, it is important that you try to make your key ring look as
much like the sample as possible.”
Demonstrate the following instructions on an extra kit:
Attach cord to key ring/lanyard (all patterns). “See if you can attach the cord to the
key ring by looking at the sample. If you need help, I will show you how.” (Observe and rate
performance. Demonstrate attaching per instructions. If person cannot copy the demonstration,
attach cord for them.) “Even up the ends of your cord.”
“Tape your key ring to the table with masking tape so it will not move while you work.”
Provide masking tape to tape key ring/lanyard with attached cord to working surface.
Island House
“Place three red beads on one cord and slide them down to the key ring.” (Demonstrate
and observe performance.) “Pick up the other cord and push it through the 3 beads in the
opposite direction. Pull on both cords to tighten the row.” (Demonstrate and observe
performance.)
“Continue to make rows in this way to copy the sample. When you are done, tie two
knots to keep the beads in place.” Observe, offer assistance, and rate performance by rating
criteria.
Parrot Fish/Yellowtail Fish
“Place two red beads on one cord and slide them down to the key ring.” (Demonstrate
and observe performance.) “Pick up the other cord and push it through the 2 beads in the
opposite direction. Pull on both cords to tighten the row.” (Demonstrate and observe
performance.)
“Continue to make rows in this way to copy the sample. When you are done, tie knots at
the ends of the tails to keep the beads in place.” Observe, offer assistance, and rate performance
by rating criteria.
Therapist’s Instructions (4.8 only)
Introduce activity: “This is a beaded key ring in the shape of (an Island house, Parrot fish,
or Yellowtail fish found in the Caribbean). All the supplies needed to complete this key ring are in
front of you. In order for me to see how you are doing today, I would like you to make one just
like this sample. Look at the sample and see if you can figure out how to do it on your own. Let
me know if you need assistance or have a question.” Observe, offer assistance, and rate
performance by rating criteria.

• 24 inch (60 cm) length of elastic cord.
• Beads: 24 white, 24 green, 8 each pink, blue, yellow flower beads
1. Tie overhand knot at end of cord, leaving 1˝ (3 cm) tail.
2. Place beads in following order on cord: 1 each white, green, yellow flower, white, green, pink
flower, white, green, blue flower.
3. Repeat this pattern 7 more times (cord will be full.)
4. Remove knot at starting end and retie two ends together to make the necklace.
Completion time: about 15 minutes.
Island House
Supplies
• 36 inch (90 cm) length of rattail cord.
• Key ring with attached lanyard hook
• Beads:
15 red, 17 pink, 2 white, 2 black, 1 each yellow, green, orange, blue flower OR
15 red, 17 green, 2 white, 2 black, 1 each yellow, pink, orange, blue flower

1. Attach rattail to small lanyard with knot as shown in figure.
2. Tape lanyard to table top with masking tape to stabilize work.
3. Place 3 red beads on one cord and slide them down to the lanyard
hook.
4. Pick up the other cord and push it through the 3 beads in the
opposite direction. Pull on both cords to tighten Row 1.
5. Continue to make rows in the same way as follows:
Row 2: 5 red
Row 3: 7 red
Row 4: 7 pink or green
Row 5: 1 pink/green, 1 white, 1 pink/green, 1 black, 1
pink/green, 1 white, and 1pink/green

Supplies
• 45 inch (110 cm) rattail cord
• Key ring with attached lanyard hook
• Beads: 2 red, 7 olive green, 1 black, 23 bright green,
15 dark blue, and 5 yellow
1. Attach rattail to small lanyard with knot as shown in figure.
2. Tape lanyard to table top with masking tape to stabilize work.
3. Place 2 red beads on one cord and slide them down to the
lanyard hook.
4. Pick up the other cord and push it through the 2 beads in the
opposite direction. Pull on both cords to tighten Row 1.
5. Continue to make rows in the same way as follows:
Row 2: 3 olive green
Row 3: 1 olive green, 1 black, and 3 olive green
Row 4: 3 bright green, 3 dark blue
Row 5: 3 bright green, 3 dark blue
Row 6: 4 bright green, 2 dark blue

Supplies
• 45 inch (110 cm) rattail cord
• Key ring with attached lanyard hook
• Beads: 2 red, 25 white, 1 black, 18 yellow, and
10 dark blue
1. Attach rattail to small lanyard with knot as shown in figure.
2. Tape lanyard to table top with masking tape to stabilize work.
3. Place 2 red beads on one cord and slide them down to the lanyard hook.
4. Pick up the other cord and push it through the 2 beads in the opposite
direction. Pull on both cords to tighten Row 1.
5. Continue to make rows in the same way as follows:
Row 2: 1 white, 1 black, and 2 white
Row 3: 1 white, 1 yellow, and 3 white
Row 4: 1 white, 1 yellow, and 4 dark blue
