TRANSPORTATION REQUEST/PERMISSION FORM
The following information is needed to help us determine bus routes for the year. Your child
may be picked up at your home, however, another pickup and drop off point may be necessary
due to bus safety law and Coldwater Community Schools scheduled stops. Michigan state law is
very specific as to the length of time the child may be on the bus and how far apart stops can be.
For our preschool children, they are placed in the front seats of the buses, and if there are 10 or
more preschool age children on the bus there is a bus monitor sitting with them. If there is a
change in this information it is the parent’s responsibility to notify the bus garage and the school.
