I am aware that my participating in the activity involves a certain amount of risk. I understand that I will have to
cooperate fully with the staff and diligently comply with the staff's instructions and all safety systems. I declare and
confirm that I have read and fully understand all the parts in this rental form (including the Rules and Regulations
overleaf) and I hereby accept the risks involved in the activity as disclosed in the information provided by PA WaterVenture. I will therefore not hold PA Water-Venture or People's Association or their servants and agents responsible
for any damage to or loss of property or any injury or loss of life which may be sustained by me during the activity or
arising from any cause in connection with the activity where such damage to or loss of property or any injury or loss of
life is not caused by the negligence or wilful act or omission of PA Water-Venture or People's Association or their
servants and agents.
I declare that all the information provided above is true. I have read the PAR-Q and/or have sought the doctor's advice
on where I am unsure and have determined that I am fit to take part in the activity.
I agree that PA Water-Venture reserves the right to use, reproduce, edit and distribute any photographs, motion
picture, recordings, or any other record of its participants for any legitimate purpose, including commercial advertising.
I have obtained permission from my parents/guardian to use the equipment and agree to the terms and conditions
above as I am under 21 years of age.

Remarks
RULES AND REGULATIONS FOR RENTAL OF EQUIPMENT
1) Rental for activities is allowed only when relevant proficiency certificates issued by People's Association WaterVenture or other relevant organisations recognised by the National Sports Associations are produced as documentary
proof where applicable. E.g. Users who intend to use closed deck kayaks must produce the relevant certifications i.e.
1 Star Kayak Award.
2) All users must put on a Personal Floatation Device and be properly attired when engaging in the water sport
activities.
3) Sharing of rented equipment is not allowed.
4) Check all equipment before use, i.e. no leaks, damages etc. Defects must be reported to the staff immediately.
5) Users would be held responsible for any damages to the equipment during rental.
6) In case of poor visibility or bad weather, such as a thunderstorm, please head to the nearest shore/shelter. In the
event that lightning activity is detected in the vicinity, the warning siren will be activated. For your own safety, please
return to the outlet immediately.
7) In the event of capsize, please check for your buddy (if any) and recover craft. If you are unable to recover the craft,
stay with the craft, seek assistance by attracting attention e.g. waving your hands or paddles, blowing the whistle.
8) Do all activities within the demarcated areas. The demarcated areas are displayed on the outlet notice board.
9) All users must strictly adhere to all safety regulations by the relevant authorities and Water-Venture.
10) Please ensure you are able to contact the office before you embark on any expeditions in the event assistance is
required. E.g. bringing along a means of communication and have the number to the Water-Venture office.
11) Keep the office informed of your intended expedition route if applicable.
12) All rented equipment must be returned and thoroughly rinsed with water before storing.
