Trilby de Jung welcomed the group. 2. Trilby provided an FLHSA update a. High Blood Pressure Registry and Regional Efforts 1. Funding for the high blood pressure initiative has previously been through a surcharge paid by the Monroe County hospitals, insurance carriers and businesses. Unfortunately, the governing group to the high blood pressure initiative (the Healthcare Planning Team- [HCPT]) has decided to temporarily pause the surcharge as they try to decide where to focus in a new program. 2. The registry will continue for the foreseeable future. However, certain programs centered in Monroe County will not (i.e. the PIC and community health worker programs). The HCPT will be providing funding support to transition the registry over to the RHIO. Albert Blankley is actively meeting with members of the RHIO to help develop this transition. There is a possibility that the registry will be able to include more information than just hypertension- such as BMI/obesity, A1C and smoking. An update will be provided to the group when one is available. 3. Trilby indicated that the governing group was considering looking at different chronic diseases, such as obesity or diabetes (in addition to hypertension) but could not reach consensus and therefore put the hold on the surcharge. b. Political Landscape Changes and Potential Impacts 1. While we can only speculate at this time, it appears based on conversations in Albany that DSRIP will remain funded for its 5-years. It is unlikely that there will be a DSRIP extension after 2020 when the grant ends. FLHSA will maximize utilization of FLPPS in the community over the remaining funding years. 2. The new HHS director will likely focus on patient empowerment and transparency. It is thought that there will be a more robust insurance marketplace than there is now. It will be very important for the S2AY Rural Health Network and their patient navigators to help community members find health insurance that fits their needs. 3. Tom Mahoney discussed the FLHSA Practice Transformation work a. An electronic version of the presentation was provided to the group. Tom noted that a “transformed practice” focuses on the patient and their growing needs, not just on treating a specific problem when it arises. To achieve success, a practice must have engaged leadership. c. FLHSA can help practices in the community by being a resource for training, coaching and mentoring. FLHSA will target the change concepts needed to improve outcomes and adapt to the value based payment models. 1. A short summary is being developed that could be provided to practices who want to learn more information. This will be provided to the group when it is available. There has been no word on the LIFT Population Health grant nor the CBO grant. S2AY was advised that the decision makers have not yet made a selection. Derrik will advise the group as soon as they hear of anything. b. *Update as of 12/8/16: The CBO grant has been awarded to S2AY. More information will be provided as it becomes available. 5. Anne Ruflin proposed the 2017 Regional Leadership schedule a. The 2017 schedule was proposed to the group. The meeting dates for 2017 are: 1. Friday March 3rd, 2017 2. Friday, June 2nd, 2017 3. Friday, September 22nd, 2017 4. Friday, December 1st, 2017 b. All meetings will continue to be held at the Yates County Office Building in the ground floor auditorium.
