An Overview of the Program and Services
The Healthy Families Program at Family Service Association of Howard County (FSA)
provides services in Clinton, Howard and Tipton County in the state of Indiana. The
Healthy Families program is a free and voluntary program offered to low income prenatal
parents and parents with children ages zero to three. The Healthy Families program
provides parents with education and support through weekly home visits.
The mission of the Healthy Families program at Family Service Association of Howard
County (FSA) is to promote supportive environments that optimize child growth and
development and encourage resilient, healthy families. The goals of the program are to
prevent negative outcomes, increase parenting skills/behaviors, increase healthy
pregnancy practices, increase ongoing health care practices, increase mental health
indicators, increase social support systems, and improve family environment factors.
Indiana State Department of Health – Women, Infant and Children (WIC) program,
Community Howard Regional Health System, St. Joseph Hospital, and St. Vincent- Frankfort
Hospital provide the main source of referrals to the program.
To participate in home visiting services, parents must enroll before their child is 3 months
old, have income below 250% of the poverty level, and have personal and/or
environmental factors that place them at risk for child maltreatment. Families are eligible
to stay in the program until their child is 3 years old.
Introduction
FSA began providing Healthy Families services in 1995. The Healthy Families Program at
FSA is a Healthy Families America affiliated program and participates in the HFA multi-site
accreditation process. The Healthy Families Program at FSA is also a Healthy Families
Indiana affiliated program and participates in the HFI sponsored multi-site accreditation
process.

Summary of highlights
The program hosted 2 picnics, 2 holiday parties, and 9 mom’s groups in 2012. With the
help of the community, the program was able to provide every child with gifts during the
holiday season. Through a grant from the city of Kokomo, the program was able to provide
Kokomo families with educational toys and books. Families in need of safe places for their
baby to sleep or a safe place for their baby to ride received pack-n-plays and car seats
through Family Service Association of Howard County. The program also offered free
individual counseling to families through Family Service Association of Howard County.
2 new staff members were trained in the “Parents As Teachers” curriculum. All staff
received updated core assessment and home visiting training.
Due to changes in the way the Indiana State Department of Health – Women, Infant and
Children program is allowed to make referrals, we no longer receive names of post-natal
referrals from WIC.
Summary of services provided
The program screened 808 referrals and assessed 298 families. Of the families assessed,
251 were determined eligible for home visiting services, and 98 new families received
home visiting services in 2012. The acceptance rate for 2012 was 39%.
121 families continued receiving home visits from 2011 into 2012. Altogether 219 families
received 3,458 home visits in 2012. Overall 81% of families received 75% or more of all of
the visits they were expected to receive, and 51% of families received 90% or more of all of
the visits they were expected to receive.
In addition, 350 Ages and Stages Development Questionnaires were given to families, and
81% of home visits were related to goals and objectives set by the family.
As comparison, in 2011 311 families were assessed and 254 families received home visits.
The decrease in home visiting families from 2011 – 2012 is partially a reflection of
improved retention in Tipton and Clinton County in 2012.

49 Clinton, 239 Howard and 10 Tipton families were assessed. 48 Clinton, 163
Howard, and 8 Tipton families received home visiting services in 2012.
Total number of families discharged
87 families were discharged from the program: 18 from Clinton, 67 from Howard
and 2 from Tipton. Reasons for termination from the program include: 8%
returned to work or school, 13% graduated, 14% aged out, 15% were unable to be
located, 21% moved, and 25% refused continuation of services.
Number of families who successfully completed the program
To successfully complete the program a family must meet the following
requirements. Parents must maintain stability in the home with no crisis or safety
issues for at least 30 days. Parents must demonstrate regular use of effective
independent problem-solving skills. Parents must demonstrate significant
reduction of risk factors identified using the initial assessment including use of
corporal discipline methods. Parents must consistently demonstrate positive
parent-child interaction skills. Parents must utilize at least two positive support
individuals. Parents must complete two goals in the past 12 months. Parents must
utilize the child’s medical home appropriately. The child’s immunizations are up to
date and the parent addresses environmental safety concerns independently.
In 2012, 11 families successfully completed the program: 1 from Clinton and 10
from Howard.
Summary of information about program staff
In 2012, the program employed 15 staff members. The program positions are as
follows: 2.0 FTE Assessment Staff, 9.0 FTE Home Visiting Staff, 2.2 FTE Supervisory
Staff, and 0.8 FTE Program Manager, 1.0 FTE Administrative Assistant and 0.50 FTE
Community Liaison. We had 100% staff retention in 2012.
Two staff are African American, three staff are Hispanic and bilingual, 11 staff are
Anglo. Seven staff have a bachelor’s degree and two staff have a master’s degree.
