WE ARE PROUD
that transition age youth have been a priority within the
system of care at Alameda County Behavioral Health Care
Services (ACBHCS) for the past seven years, and that we
enthusiastically support the Mental Health Services Act.
WHAT IS THE TAY SYSTEM OF CARE?
The Transition Age Youth (TAY) System of Care exists to improve the
services and outcomes for youth, age sixteen through twenty-four who are
experiencing mental illness, in making successful and seamless transitions
towards self-sufficiency and independent living. WHO DO WE SERVE?
The TAY System of Care primarily serves youth aged 16-24 who are Seriously Emotionally Disturbed (SED) or
Seriously Mentally Ill (SMI). As a result of the mental disorder the young person has substantial impairment in
self-care, school functioning, family relationships, or in their ability to function in the community.

Including youth in the planning, delivery and evaluation

Creating services based upon the Wellness,
Recovery and Resiliency model;
Accepting a young person where he or she is at and
supporting positive changes from that point;
Ensuring that youth in all communities have access to
the same quality of care.
The Mental Health Services Act (MHSA) passed by voters in 2004, mandated a change
in the way mental health services are delivered. Consumers are invited to be partners in
their care and for the first time it seems there is real hope that a person can recover from
mental illness and go on to live a happy, productive life. Youth exiting children’s services who will need assistance transitioning into adult life, avoiding
hospitalization, homelessness, or incarceration;
Youth currently receiving adult mental health services who need more and age-appropriate support;
Youth with no mental health service history or those who have only had contact with county
crisis programs.

go beyond office-based treatment plans may be referred
to the TRANSITION ASSESSMENT TEAM by calling:

In addition, there are many other youth-based services available to meet the
mental health needs of Transition Age Youth in Alameda County.

Berkeley’s Mental Health Division and
Alameda County.
Bay Area Youth Centers (BAYC) is a Hayward
based, long-term residential program that
serves adolescents in foster care.

comprehensive services that encompass the
mental, emotional, physical and social
dimensions of development.
