The San Joaquin County Commission on the Status of Women created the Susan B.
Anthony Women of Achievement Awards to honor women who have excelled in their
chosen category, and have, through their dedication, talent and untiring efforts raised the
status of humankind in San Joaquin County.
Nomination packets are now available for individuals, organizations, and businesses to
place women’s names in nomination. Award candidates must be current residents of San
Joaquin County or be employed full time in the county. There are 18 categories in which a
candidate may be nominated.

Please mail the 12-page (max) nomination Packets to SJCCSW, PO Box 4443,
Stockton, CA 95204.
The nomination Packets will be picked up at 5 p.m. on Thursday , December 31.
Late deliveries will not be accepted. The judging will take place over the weekend.
Award winners will be notified by phone the following Monday.
The 42nd Annual Susan B. Anthony Banquet to Honor Women of Achievement will be
held on Monday, February 15, 2016 at the Stockton Golf and Country Club, 3800 W.
Country Club Blvd., Stockton, California at 5:30 p.m.
Tables of 10 are $500 (one check please) and due by Friday, January 29.. Individual
tickets at $50 are due by Monday, February 1st. All meals must be paid in advance – no
tickets will be sold at the door. Your canceled check is your receipt. Please include the
names of the attendees for nametags along with the checks.

NOMINATION PACKAGE
General Information for 2016 SBA Award Nominations

Criteria
Only the candidates whose “volunteer efforts” or “professional work plus volunteer
efforts” to have raised the status of humankind in San Joaquin County will be
considered.
Award candidates must be current residents of San Joaquin County or full-time
employees in San Joaquin County
The candidate must have excelled in her category.
Only the candidate’s efforts documented within the application package will be
scored.
No more than ten women will receive the award in any single year.
An additional Young Feminist of Achievement award may be granted.
A woman may receive only one Susan B. Anthony Woman of Achievement award in
her lifetime.
The candidate must be nominated by an individual, organization, or business (hereafter
referred to as the “nominating organization”).
The nominating organization is responsible for submitting the following documents in a
timely manner on or before the cut off date.

The 12 page application must include:
1-page Nominating Organization Cover Letter: (see next page)
Multi-page Candidate’s Statement completed by the candidate giving her name and
contact information, signature, and the answers to 4 questions
Candidate’s Supporting Category Documents: may include, but are not limited to, a
resume, letters of recommendation, newspaper clippings, photos, lists of awards and
certificates, and thank you letters. Please do not include originals or copies of the awards or
certificates.
Mail application package to arrive in the PO box by 5 pm, December 31, 2015.

Award Announcement
The Selection Committee will meet the weekend of January 2nd – 3rd.
Award recipients will be notified by phone by the following Monday, Jan. 4 by the Chair of
the Selection Committee or by the Chair of the San Joaquin County Commission of the
Status of Women.
Non-recipients and all nominating organizations will be notified by mail the following
week.
SBA Dinner
The awards dinner will be held Monday , February 15, 2016, with reception at 5:30 PM and
dinner at 6 PM at Stockton Golf and Country Club. Tables seat 10 people each.
Honorees’ dinner and award, $75, are to be paid by the sponsor individual or organization. Each
honoree may designate up to nine guests to sit at her table. When more than one nominator
has nominated the honoree, it is customary to share the costs between/among the nominators.
All other dinner guests’ dinners are $50. Due to the large number of guests and organizations
attending the dinner, additional honoree’s guest tables may not be located near her awards
table.
Program: Honorees are introduced by the mistress-of-ceremonies and presented their
SBA medal. The Honorees then have three minutes to address the audience on any
aspect of their life they wish to share. Half-page details of each honoree’s winning
accomplishments will be summarized in the printed programs.
Reservations: by Friday, January 29 th. Please submit a maximum of one check per table
of 10. Please submit the recipient’s fee, all guests names (designate the 9 guests to join
the honoree at her table), and the $50/each guest dinner fee to:
