Thank you for your work on behalf of Nebraska children! You are being contacted because we hope you will
participate in a short, on-line survey on Child Care Market Rates in Nebraska. This survey is being conducted for the Nebraska Department of Health and Human Services (DHHS) by the Nebraska Center for Research on Children, Youth, Families and School (CYFS) at the University of Nebraska-Lincoln.
The survey takes approximately 8-10 minutes to complete.
We would like one response per child care licensee. If you hold more than one child care license, please
complete for each license.
This important survey is conducted to meet the requirements of state and federal law. Results will be used
to help determine the reimbursement rates for Child Care Subsidy. Your response is central to establishing
this reimbursement rate.
Please be assured that all answers will be kept strictly confidential and you will not be penalized in any way
for your participation or withdrawal from participation. You are not required to participate and your licensing
status will not be affected by your response.
