AFINITRAC Care Champions are all registered nurses, each with
an average of almost 20 years of experience in areas such as
oncology, nephrology, and pediatrics. When you register for
AFINITRAC, you will be matched with one of our Care Champions
who will provide you with ongoing, personalized support.
Care Champions provide:
» Regular check-ins
» Treatment support that is tailored to you, including
- Education about treatment
- Assistance working with government agencies,
such as social services
» Coordination of prescription refills and renewals
» Full translation services in over 160 languages
- Translation support available Monday through Friday,
8:30 am—5:30 pm ET
