Nyborg, Roselle ­ public accountant license (065­007441) suspended for failing to file and/or pay Illinois sales tax and for failure to file income tax returns. Herbert P. Wasserman, Chicago ­ public accountant license (065­007560) indefinitely suspended for failure to make a substantive response to a Department request for information regarding a complaint from a former client. ARCHITECTS, LAND SURVEYORS, PROFESSIONAL ENGINEERS AND STRUCTURAL ENGINEERS Marvin Holt, Lombard ­ (unlicensed) ordered to cease and desist the unlicensed practice of architecture. William Olen, Des Plaines ­ (unlicensed) ordered to cease and desist the unlicensed practice of architecture and to pay a civil penalty of $3,000 after he produced architectural drawing for three separate residential construction projects in Illinois, then used the seal of a licensed architect on the plans without authorization. The plans were not prepared by a licensed architect or under the direct supervision of a licensed architect. ­ 1 ­ BARBER, COSMETOLOGY, ESTHETICS AND NAIL TECHNOLOGY Jill A. Kohut, Naperville ­ cosmetologist license (011­220023) reprimanded and fined $250 for failure to provide valid proof of continuing education hours. Sago, Chicago ­ cosmetologist license (011­262890) placed in refuse renew status for being more than 30 days delinquent in the payment of child support. James Brian Sanders, Chicago ­ cosmetologist license (011­165757) revoked due to a felony conviction. Algela C. Brown, Chicago ­ nail technician license (169­020357) issued on indefinite probation for a minimum of five years due to a felony conviction. David Nguyen, Carol Stream ­ nail technician license (169­018527) placed on indefinite probation after defaulting on his education loan and entered into a repayment agreement. Glamour Nail, Calumet City ­ salon license (189­003862) placed on indefinite probation for a minimum of five years due to unprofessional conduct and aiding and assisting unlicensed practice of numerous individuals. Norman Neil Brown, Parkland, FL ­ physician and surgeon license (036­096331) was reprimanded for gross negligence after performing a wrong level anterior cervical fusion. Glenn M. Charles, Boca Raton, FL ­ physician and surgeon license (036­095038) indefinitely suspended after being disciplined in the state of Minnesota. Lawrence L. Ciccarelli, Mount Vernon ­ physician and surgeon license (036­087282) revoked and fined $10,000 for prescribing a prescription over the internet without a physician­ patient relationship to a person he never met or examined and without reviewing medical records or history. Craig Copeland, Livonia, MI ­ physician and surgeon license (036­091852) restored to probation to terminate on June 30, 2007. Gordana Ivanovic, River Forest ­ physician and surgeon license (036­078459) and controlled substance license (336­041523) indefinitely suspended due to unprofessional conduct and habitual or excessive use of controlled substances by appearing at her office while intoxicated and appearing at another healthcare facility while intoxicated. NURSING Sherry Bailey, Bloomington ­ registered nurse license (041­279628) reprimanded due to diversion by false prescription. Lori Carter, Lincoln ­ registered nurse license (041­233576) automatically and indefinitely suspended for a minimum of six months after violating the terms and conditions of her consent order. Joseph Dutton, Cave­in­Rock ­ registered nurse license (041­238805) reprimanded after writing unauthorized prescriptions for patients without the physician’s knowledge. Regina Y. Easterling, Chicago ­ registered nurse license (041­268309) indefinitely suspended for failure to comply with her probation regarding repayment of her educational loan. License then restored to indefinite probation after entering into a new repayment agreement. Melanie Hayse, Mesa, AZ ­ registered nurse license (041­302190) suspended for three years, followed by probation for two years after being disciplined in the state of Arizona.

Amber Michelle Bonomo, Danville ­ pharmacy technician license (049­137818) revoked after diverting controlled substances from her pharmacy employer in order to sell them to another individual for monetary gain. Hakeem M. Ishola, Jr., Country Club Hills ­ pharmacy technician license (049­ 099680) placed in refuse to renew status for being more than 30 days delinquent in the payment of child support. Nick Magas, Chicago ­ pharmacy technician license (049­134629) indefinitely suspended for failure to file and/or pay Illinois state income taxes. DS Pharmacy, Inc., d/b/a Drugstore.Com, Bellevue, WA ­ pharmacy license (054­ 014057) indefinitely suspended after failing to submit the required application and fee for a change of pharmacist­in­charge, failing to submit the application for a pharmacy change of address, and failing to comply with a written request made by the Department within 60 days.

Hook SuperX, Inc., d/b/a CVS Pharmacy 6370, Mount Vernon ­ pharmacy license (054­011663) fined $1,500 for operating without a pharmacist­in­charge from April 27, 2006 through June 15, 2006. PHYSICAL THERAPY Bradley J. Umfress, Grayslake ­ physical therapist license (070­006562) placed on probation to run concurrent with criminal probation scheduled to terminate on October 5, 2009 due to a felony conviction of Attempted Aggravated Battery of a Child. Mitigating evidence revealed child’s injuries sustained from accidental fall during a feeding session with Respondent. PODIATRY Carol A. Tucker, Chicago ­ podiatrist license (016­004269) indefinitely suspended for failure to comply with the conditions of probation regarding the repayment of her educational loan. PROFESSIONAL COUNSELORS Kevin McClure, Mackinaw ­ licensed professional counselor license (178­003869) placed on probation for one year due to unprofessional conduct related to billing irregularities and practicing beyond the scope of licensure.
