Jeff Jordan was a 4X Ohio State Champion, Big Ten Champion and 2X All-American at the University of Wisconsin. Coach Jordan spent 5 years coaching at Purdue University before moving back to his hometown in
Ohio. Coach Jordan has since led St. Paris Graham High School to 13 straight Div. II State Championships as
well as 2 National Championships during that time. He has been named National Coach of the Year on two
occasions. Coach Jordan has coached many wrestlers that have gone on to become NCAA All-Americans, including current Penn State wrestler and defending National Champion David Taylor. Wrestlers from all over

Parent Consent and Waiver of Responsibility
In consideration of St. Stephens High School, Saint Wrestling Club, Jeff Jordan, and other coaches involved
with the camp, acceptance of the camper named above as a wrestler in the camp, the camper by and through
his parent or legal guardian hereby acknowledges, understands and agrees to as following:
Wrestling is a sport, which involves intense physical contact between two Individuals. The camper will be involved in some intense training and competition including competitive wrestling. Injuries can and do occur
during wrestling. As parent(s) or legal guardian(s), we have also been informed that various skin conditions
are preventable in the sport of wrestling and while strong measures will be taken to prevent the spread of skin
conditions such as RingWorm, Herpes, and Cold Sores, 100% prevention can not be guaranteed. Further, we
the parent(s) or legal guardian(s) have been informed that there is an assumption of risk when anyone participates in the sport of wrestling. The understanding on behalf of themselves and their child or ward agrees to
hold harmless St. Stephens High School, Saint Wrestling Club, Jeff Jordan and coaches, from and against
any injuries incurred by the camper. The understanding hereby releases, waives, and forever discharges Catawba County Schools, St. Stephens High School, Saint Wrestling Club, Jeff Jordan and coaches from and
against any and all claims, injuries, demands, actions, or cause of actions arising out of the participation by
the camper in this camp. The understanding hereby certifies that the camper is physically able to participate
at the camp and that there are no impairments that would limit the participation in the programs. The understanding hereby grants permission for doctors and their designs to administer appropriate medical care, antigens, or injuries, and to perform emergency procedures as necessary.
I hereby consent for my son to participate in the 2013 Jeff Jordan State Champ Camp at St. Stephens High
School hosted by the Saint Wrestling Club. I do attest that my son, ___________________________, is in
good physical health and have no knowledge of any reason why he cannot participate in strenuous activities.
By signing, I do hereby release Catawba County Schools, St. Stephens High School, Saint Wrestling Club,
Jeff Jordan and all coaches involved with the camp from any liability from participation during this camp.
