III.
DESCRIPTION OF FINDINGS & CONCLUSIONS
A. Description of Facility
This residence is a 3 bedroom, 2 bath home located on a residential street in Warren,
Mi. The home has city water and city sewage service.
The following rooms were inspected and approved for day care use:

The only other room that will be used occasionally for napping infants is the licensee’s
son’s bedroom. An infant will be placed in a porta-crib when sleeping in the licensee’s
son’s bedroom.
There is a total of 456 square feet of available indoor activity space. Day care children
will use the bathroom in the basement. The basement has an egress window that
meets the requirements of Rule 1943 (12). The backyard is completely fenced in.
There is a Little Tyke slide set that will need cushioning materials underneath it before
children will be able to use it. The licensee will not allow children to use the slide set
until the materials are obtained and placed underneath it.
There are no pets or weapons in this home. There is an electric fireplace in the
basement that is blocked off by a baby gate.
This day care home is well equipped with toys, materials and furniture for 12 children at
any one time.
B. Program Description
The licensee has had a family day care home at this address since July 2005. She has
had no complaints filed against her home. This day care home operates Mondays thru
Saturday from 6:30 a.m. to Midnight. Mrs. Townsend has taken great care in training
her assistants. She devised a very thorough staff-training handbook to insure her day
care children receive the best of care.
Mrs. Townsend has current CPR and First Aid. She is on the Federal Food program
and is active in the local provider association group.

C. Rule/Statutory Violations
R400.1907
Children’s records.
(1) At the time of initial attendance, the caregiver shall obtain
the following documents:
(a) A completed child information card on a form provide by
the department or a comparable substitute approved by
the department.
(b) A child in care statement/receipt using a form provided
by the department and signed by the parent certifying
the following:
(i) Receipt of a written discipline policy.
(ii) Condition of the child’s health.
(iii) Receipt of a copy of the family and group child care
home rules.
(iv) Agreement as to who will provide food for the child.
(v) Acknowledgement that the assistant caregiver is 14
to 17 years of age, if applicable.
(vi) Acknowledgement that firearms are on the premises,
if applicable.
(c) Documentation that immunizations and boosters, as
recommended by the department of community health,
are any of the following:
(i) Have been completed.
(ii) Are in progress.
(iii) Are not being administered due to religious, medical,
or other reasons based on a waiver signed by the
parent.
(d) If a parent objects to emergency medical treatment on
religious grounds, the parent shall provide a signed
statement that he or she assumes responsibility for all
emergency care.
Five child information cards were incomplete and several OCAL 3900 forms were
outdated.

Smoke detectors; fire extinguishers.
(3) A home shall have at least 1 functioning multipurpose fire
extinguisher, with a rating of 2A-10BC or larger, properly
mounted not higher than 5 feet from the floor to the top of
the fire extinguisher, on each floor level that is used by
children in care.
The fire extinguishers on both levels were more than 5 feet from the floor to the top
of the extinguisher.

Outdoor play area and equipment.
(5) When swings, climbers, slides, and other similar play
equipment with a designated play surface above 30 inches are
used, they shall:
(a) Not be placed over concrete, asphalt, or a similar surface,
such as hard-packed dirt or grass.
(b) Be safe, in good repair, and age-appropriate.
(c) Be placed at least 6 feet from the perimeter of other
play structures or obstacles.
The Little Tyke slide set was higher than 30 inches and it was placed over grass.
IV.
RECOMMENDATION
I recommend issuance of a provisional license to this child day care group home
(capacity 7-12).
