2016-2017 OPENING SCHOOL INFORMATION
School Registration will be held at Mt. Carmel Middle
School located at 1520 Poplar Street for students attending
South Elementary School, North Intermediate Center
of Education, and Mt. Carmel Middle School on
Tuesday, August 2nd from 9:00 a.m. – 1:00 p.m. and 3:00
p.m. – 7:00 p.m. Please enter through the event entrance
to register/pay fees and receive class assignments and
school supply lists. South, NICE and MCMS class lists
will be posted at MCMS on Registration Day.
Parent(s)/guardian(s) must accompany students to
registration to sign required forms. Please bring your
child’s Social Security Number; kindergarten and
other new students need a certified copy of their birth
certificate. Student profile information and emergency
information will be updated

these registration times will not be able to register until

Mt. Carmel High School students will register at Mt.
Carmel High School located at 201 Pear Street:
Seniors: Tues., August 9th …8:00 a.m. – 9:30 a.m.
Juniors: Tues., August 9th …10:00 a.m. – 11:30 a.m.
Sophomores: Tues., August 9th …1:30 p.m. – 3:00 p.m.
Freshmen: Tues., August 9th …4:30 p.m. – 6:00 p.m.

Fee Schedule:
1) Kindergarten…$54.00; 2) South 1-2…$59.00;
3) North 3-5…$74.00; 4) MCMS 6-8…$77.00 (optional
PE shirt/shorts-$20); 5) MCHS 9-12…$65.00 plus
workbooks and lab and shop fees; (Driver Education fee$75).
Pay to Play: MCHS Athletes…$75 per sport
MCMS Athletes…$50 per sport
Season tickets: MCHS Activities…$50 for students;
MCMS Activities…$35 for students;
All School Activities…$100 for non-student individuals;
All School Activities…$150 for non-student couples.
Cafeteria prices:
Breakfast MCMS, North, South =$1.00
Adults =$1.25
Lunch MCHS, MCMS, North, South=$2.50 Adults =$3.00
Registration and activity fees MUST be paid at the time
of enrollment by cash, check, or contract. Fee waiver and
free/reduced lunch information will be available during
registration and/or after July 1st on the district website.
Please bring food stamp case ID number OR gross
income information.
All students will receive school time accident insurance
coverage at no cost during the 2016-2017 school year.
This coverage is for school-time activities, practices, and
interscholastic competitions ONLY and can be used as a
supplement to your existing insurance plan. A 24-hour
accident and/or football coverage insurance plan will also
be available for purchase.
HEALTH REQUIREMENTS
Prekindergarten: lead screening verification, completed
physical, up-to-date immunization record.
Kindergarten: lead screening verification, completed
physical, up-to-date immunization record, vision exam,
dental exam.
2nd grade: dental exam.
6th grade: completed physical, up-to-date immunization
record, dental exam.
9th grade: completed physical, up-to-date immunization
record.
12th grade: up-to-date immunization record
New students from out-of-state: completed physical, upto-date immunization record and vision screening (before
registering for school without exception). New students
should call or visit the school of attendance to preregister.
TRANSPORTATION
All students are required to fill out a Bus Transportation
Request Form at registration. A student must live at least
one and one-half miles from the attendance center in order
to be eligible to ride.
Monday, August 15th will be the first full day of classes
for students; buses will run; and lunch will be served.
Meet and Greet Night: Parents/students may get
acquainted with classroom teachers, bring school supplies,
purchase lunch tickets, and check out their classroom on:
SOUTH: Aug. 11th - 4:00-5:30 p.m.
NICE:
Aug. 11th - 5:00-6:30 p.m.
MCMS: Aug. 11th - 5:30-7:00 p.m.
