to participate in the Medina Library’s Teen Mini Con program on Friday Dec. 16, 2016 from
6:00pm-10:00pm.
I understand that my teen will be participating in an after-hours library lock-in after the library
has closed for the evening and that the public will not have access to the building. I grant them
permission to participate in all activities.
I understand that this is a public program and photos may be taken during the event that could
appear on the library Facebook page or in local newspapers, or be used in library promotional
materials.
I understand that my teen will be given rules and asked to follow them. If my child misbehaves
or breaks any of the rules, I understand that any Medina Library staff has the right to ask my
child to leave the library. If my child is asked to leave I understand that I will be notified and
that I will be expected to pick them up immediately. I have read the attached rules and
understand what is expected of my teen.
In addition, I understand that my teen is responsible for their own belongings and personal
property should anything be lost or damaged during this program.
In case of an emergency or if my child needs to be picked up, I would like Medina Library to

Teen Mini Con Rules
1) Please DO use your school’s dress code guidelines for your costume.
2) PLEASE do NOT bring any weapons or violent toys as part of your costume.
3) Any library staff member has the right to confiscate any part of your costume.
4) DO listen to all library staff and volunteers and do as you are asked!
5) DO be kind and take turns with your fellow anime fans during the Wii gaming session!
6) DO stay out of staff areas of the library during this event.
7) DO HAVE FUN and ask any staff member if you have questions!
On the night of Mini Con, you must check in at the
Registration table by 7:00 pm.
After 7:00 pm, you will not be able to enter the library.
