GE Dishwasher Rebate Program – DW12 (Canada)
To receive this rebate, you must:
1. Choose the recall remedy rebate option instead of the repair option. You
may not have both.
2. Complete the bottom portion of this certificate, providing your name, address
and phone number, along with the rebate option you have selected.

dealer that meet all conditions outlined
3. Attach the sales receipt from your new dishwasher to the certificate. Receipts
must show date of purchase on or after the recall announcement date and
include your retailer’s name, your name, address and new dishwasher model
number. New dishwashers must be purchased from an authorized Mabe GE
retail dealer or participating Authorized Builder Distributor. This rebate DOES
NOT apply to purchases of used or as-is products.
4. Record the model number/serial number from your recalled
dishwasher in the space provided below. If the installer of the new
dishwasher will not be hauling the old model away, be sure to
properly discard it according to local product disposal regulations
(Dishwasher); your recalled dishwasher must not be re-used.
5. Print your old model/serial number in the box below, include your new
dishwasher sales receipt and mail this cash rebate certificate to:

6. Your rebate check will be mailed to you in six to eight weeks. The check is
valid up to 90 calendar days after issuance.
7. The rebate can be used in conjunction with any other retail sales promotion.
8. If you have not received your rebate after eight weeks or you have additional
questions, please call the RECALL HOTLINE at 1-888-943-5188 Monday
through Friday (8:00 a.m. – 5:00 p.m. eastern time.) Before calling, you should
have the new unit’s model and serial number on-hand.
