Remember to fill in the day and date. It is important to note the time you eat each day. Were you hungry? Say how empty or full you felt (1 = full / 10 = very hungry) What did you eat or drink? Include everything even the odd biscuit, sweets and drinks. How much? Give estimated portion sizes – there is no need to weigh anything. Remember to note how it was cooked. What did you add? Was it grilled, fried, boiled? Record any activity in your day, what you did and for how long. Don’t forget daily activities such as housework, washing the car, gardening. It all counts! Instructions: Many people find it helpful to keep a record of what they eat and drink. Include a description of how strenuous you found the activity e.g. “brisk walking”, “heavy gardening”, “light housework” Doing this will help you be aware of what, when and why you eat, and from this, you can make some healthy changes to your lifestyle. This is your own personal record and you do not need to share it.
