Middle School Afterschool Enrichment 2016-2017 School Year Contact Information Emmalie Wilcox (402)-323-6433 [email protected] Teen Time In our after school enrichment program Teen Time, for Schoo Middle School Students. participants will benefit from a safe place to stay after school that includes a nutritious snack, academic support, personal development, recreational and social time and organized activities. Students can participate in other after school clubs and intermural sports and then participate in the Teen Time program. Fees Registration Fee $20/Child Y Family Member Y Youth Member Non-Member $25/Month/Child $100/Month/Child $150/Month/Child The monthly program fees are based on the total number of school days August through May averaged over 10 months. Monthly program fees will not be pro-rated during months which include extended breaks. Activities Activities include: arts & crafts, indoor & outdoor sports, music, nature, swimming, cooking club, Chill Time (board games, Wii & Xbox Kinect games, ping-pong, foosball), homework time, field trips, drama and more. There may be days involving field trips when the groups will return to the school site by 5pm. Parents will be notified in advance by posted messages, flyers and/or in the activity schedule. During these activities, alternative care will not be available at the site. Please plan for your child to attend field trips, or arrange to have your child picked up directly from school on these days. Snacks A nutritious snack is provided each afternoon. Transportation Options Students participating in Teen Time who live in Air Park and qualify for a regular bus ride to school have the option to be transported home on the 5:30 pm late bus. Fallbrook Y Youth Policy When Lincoln Public School is in session throughout the school year, anyone 14 years of age or younger must be participating in a supervised program or be accompanied by a parent, or responsible adult, in the Fallbrook YMCA facility, between the hours of 3:00 PM and 5:00 PM, Monday to Friday. Teen Time Location & Pick– Up Procedures The Teen Time program is held in the shared space between the Fallbrook YMCA and Schoo Middle School. Students should always be picked-up at door #18 on the South Side of the building. We ask that you come in to sign your student out of the Program. If you wish to have your student sign themselves out of the program to walk out to meet you, to ride the bus home or walk home, we ask that you complete an Alternative Pick-Up Form. Late Arrival & Absence We expect that communication will be initiated by the parent/guardian and/or the student regarding attendance, which includes late arrivals or absences. Please notify the Program Staff by calling the program phone number (leave a message if there is no answer) if the student will be absent or late. There will be no reduction of fees due to absences/late arrivals. Teen Time Phone Number All of our Youth, Teen & Family Programs have a phone on site during the program. Photo/Video Disclosure Statement The YMCA of Lincoln, Nebraska may on occasion take photographs and/or video of its members or program participants for use in printed materials or by electronic methods. Your entry into YMCA facilities, participation in YMCA programs or participation in YMCA events grants permission for the YMCA of Lincoln, Nebraska to use these photographs and/or video in its marketing and public relations efforts. Registration Information Priority Registration is ONLY for those students currently enrolled in the 2015-2016 school year program who have a Y Family Membership (any type). Y Family Member registration is for those families who do not have a child currently in the program. Open Registration is for Non-members or anyone else that wants to register. All families who want to register must have a zero balance on their YMCA account. If there are no openings in the program after Priority Registration, families who wish to be added to the program waitlist can contact the Fallbrook YMCA to be added to the waitlist on a first-come, first-served basis. We encourage you to check our website, ymcalincoln.org, under Registration Dates and News on the homepage, after February 9, 2016, to check availability and for other updates. Required Registration Materials At the time of registration you will be required to provide the following information: • Complete 2016 Child Before/After School Enrollment Form • Complete Program Direct Draft Authorization– MUST be completely filled out, including the card number • Complete 2016 Child Information Record (both sides) • Current Immunization Record (or must be on file) • Title XX Authorization, if applicable • $20/child Registration Fee Your registration will not be accepted unless all of the required paperwork is turned in at the time of registration, including the non-refundable/non-transferrable $35 registration fee. Enrollment Forms and other Required Registration Materials must be submitted, in person, at the Fallbrook YMCA Front Desk. Paperwork that is “dropped” at the Front Desk, faxed, emailed or mailed will not be accepted and your child/children will not be registered. Payments & Financial Assistance The Lincoln YMCA requires monthly program fees be automatically withdrawn from a credit or debit card. Payments must be set up for a monthly credit/debit card draft. We accept Visa, MasterCard, and Discover. Monthly deduction will begin August 1, 2016. 2016 If your payment information changes at anytime please visit the Fallbrook YMCA Front Desk to complete a new Direct Draft Authorization and Program Change/Withdraw Form. The $35 registration fee and August program fee will be due in full if registration is received after August 1, 2016. Title XX is accepted. The parent/guardian is responsible for the $35 registration fee, per the Title XX agreement between the Lincoln Y and the Nebraska DHHS. Proof of Title XX authorization and registration fee are required at the time of registration. Families responsible for a Family Fee per their DHHS authorization for Title XX benefits, must pay the Family Fee by the 1st of each month. Families who utilize this service are subject to cancellation and no-show fees. For more information about the fees and policies, or if you have questions, please contact the program director. Your membership status at the time of registration determines the fee for the program. If your membership status changes, you must visit the YMCA to complete a Program Change/Withdraw Form at least two weeks prior to the end of the month to amend your fees. Financial assistance is available for those who qualify, please visit ymcalincoln.org or any Lincoln Y for an application. Please allow 15 business days for processing. Cancellations & Membership Changes Your membership status at the time of registration determines the fee for the program. If your membership status changes, you must visit the YMCA to complete a Program Change/Withdraw Form at least two weeks prior to the end of the month to amend your fees. A Program Change/Withdrawal Form is required for any change or cancellation at least two weeks prior to the end of the month.
