HOW THE
PROGRAM WORKS
Thank you for your interest in the Heart and Soles walking
program, sponsored by Evangelical Community Hospital.
Maintain a Healthy
Body Weight
Keeping those extra pounds off reduces the risk of
heart disease as well as osteoporosis, breast cancer and
colon cancer.
The Heart and Soles program is a fun and healthy way to get
started on a free fitness program.
Get Informed
As a Heart and Soles member, you will be responsible
to record all continuous miles walked. You will receive a
membership card/mileage log in your new member packet.
Simply log your miles on your membership card every time
that you walk. When the card has been filled, or when you
reach your prize goal, turn in your card at the Susquehanna
Valley Mall Management office or Evangelical’s Community
Health Education Center and receive your incentive prize and
a replacement card. Prizes will be given to participants who
reach 100, 250, 500, 750, 1000, 1500, 2000, and 2500 (grand
prize) miles. Prizes will be provided by Evangelical Community
Hospital, and are subject to change without notice.

Heart and Soles Walking Program members will enjoy
advanced notice of special events, including presentations
by Evangelical Community Hospital experts on a variety of
health and wellness topics. Members will also have access
to free blood pressure and blood sugar checks.

I understand that participation requires regular sessions of walking on
my own schedule. I further understand that there is a risk of injury
involved in performing any type of physical activity. Therefore, if I
have any medical conditions which may compromise participation
in this program, I acknowledge that the condition has been discussed
with a physician and medical clearance for participation has been
obtained. I hereby release Evangelical Community Hospital and
the Susquehanna Valley Mall, Selinsgrove LifeTrail, T.I.M.E., LARA,
Playworld Systems, their agents, employees and representatives, from
any and all liability which may arise from my participation in the
Heart and Soles Walking Program.

The Susquehanna Valley Mall opens its doors for the
Heart and Soles program from 8–10 am, Monday–
Saturday and 10 am–noon on Sunday. Walkers should
use the Bon-Ton entrance in the front of the Mall.
