I commend our team as they fulfill our Mission of providing exemplary service to our patients and their loved ones, always striving to keep our patients needs first. After 20 years as Executive Director, I see our industry and market adversely impacted by regulatory and reimbursement changes. We remain committed to providing exemplary service to patients, and their loved ones, who are coping with the challenges of life-limiting illness, and to provide outstanding community education on issues of death, dying, quality of life, and bereavement. In March 2016 our agency partnered with Multi View, Inc. (MVI) to assist us in our endeavors. MVI was formed to provide extraordinary management systems and quantified “Best Known Practices” exclusively for the Hospice & Homecare world. They provide high-value benchmarking which assists us in finding our financial balance while continuing to provide exceptional care. In addition, we as an organization are dedicated to provide our team the opportunity to complete their certification in Hospice and Palliative Care. We are currently a certified hospice agency through Medicare and we are taking this one step further by attaining our deemed Medicare Certification through Community Health Accreditation Partner (CHAP). Through CHAP we are held accountable for Service-Specific Standards of Excellence. During our fiscal year from August 1, 2015 through July 31, 2016 we provided direct patient care to 301 patients and families throughout Gooding, Jerome, Lincoln, and Twin Falls counties. In addition our bereavement services connected us to 463 individuals and families. As you will read in this letter we go above and beyond focusing on our special programs with our Veterans and helping make dreams come true. Our promise to you and our community is to continue adapting to health care changes while focusing on the patient and family as the center of what we do. As an organization we are attentive to our roots and devoted to ensuring our existence well into the future. We realize that we only get one opportunity to provide our patients and families an extraordinary end-of-life experience and that is what we strive to achieve. Our compassionate care and specialized patient and family focus could not exist without the generous support of our team, volunteers, Board of Directors, families and friends and the community as a whole. The world of volunteerism for Hospice Visions, Inc has never looked brighter. It is our great fortune to have 27 of the finest people in South Central Idaho who have chosen Hospice Visions, Inc., to volunteer for. Thanks to their heart and dedication, our organization has consistently recognized volunteer cost savings that keep Hospice Visions, Inc. in compliance with the Conditions of Participation (CoP’s) set forth by Centers for Medicare Medicaid Services (CMS). Numbers and CoP’s are just mechanics. The true success of our volunteer program lies in the heart and soul of each volunteer. For instance the volunteer who takes his patients a root beer float or handmade lap blanket, or the Veteran Volunteer who takes one of his Veteran patients a hat specific to the job of that patient when he was in the military, that means so much that when presented with the hat, the patient sits up in his wheelchair and sings the Marine Corps Hymn “From the Halls of Montezuma, to the Shores of Tripoli” in its’ entirety…mind you this patient normally talks in word salad, nonsensical speech due to his dementia. Then there’s the tried and true Visions treasure who has been volunteering since the beginning of Hospice Visions, Inc., with her countless hours of administrative assistance and Bereavement Mailings whose value cannot possibly be calculated. A special note of thanks and recognition goes out to all the Hospice Visions, Inc.’s employees who volunteer their time at each of our special events and fundraisers, we couldn’t do any of it without their help. This is how we can truly measure our success and our accomplishments as an organization. One of our younger, very private patients’ needed a ride to a local mortuary to discreetly make her final arrangements. Another of our patients’ wanted so badly to go to the Twin Falls County Fair one last time, and yet another one of our patients’, who could not hear or see well longed for someone to just sit and be with her. A number of our patients’ simply want someone to talk to, share their memories with, or laugh with. Many of our patients would like to capture their life story on a video that can be shared with their family for generations to come. All these requests, desires and longings came true and were made possible and were fulfilled…by Volunteers. Volunteers are people who willingly give of themselves for the benefit of others. While the act of volunteering can be very rewarding and satisfying to the volunteer and we hope that it is, very few people on earth allow their goodness to be harvested in this wonderful, selfless way. We are very thankful that our volunteers have allowed us the privilege of harvesting their love and goodness. There’s always room on our team for more volunteers. Page 3 Veteran News Meeting The Needs of Our Veterans Hospice Visions, Inc., is honored to be a four star member of the “We Honor Veterans” Partnership. A partnership program between our Hospice organization and the Veterans Administration designed to give Veterans on Hospice the highest honor, respect and personal recognition they deserve as a Veteran facing the end of their lives… lives dedicated to the liberty and freedom of the United States. Numerous staff members at Hospice Visions, Inc ., are or have served in the military and many more of us have fathers, brothers, sons, and daughters and spouses who have or are currently serving in the military. We consider it a privilege to honor America’s Hero’s that choose Hospice Visions, Inc., as their Hospice provider. In addition to the behind the scenes comfort, care and love we give our Veteran patients and their families we have been publicly busy showing our support. A flag pole was installed and dedicated to the past, present and future Veterans at Rosetta Assisted Living Center. If you have any suggestions as to where our next flag pole should go please contact our office. One of our Veteran patients was an Army Air Corps Flight Instructor and when asked what it was that he most wanted before he passed, he said “to fly one more time”. Thanks to our staff and Mike Parkes of Parkes Magic Valley Funeral Home his wish came true. On June 11th we sponsored the Second Annual Vietnam Veteran Commemorative Welcome Home Motorcycle Ride Event at the Twin Falls City Park. There were 80+ motorcycles that took part in this 1.5 hour ride through the cities of Twin Falls, Kimberly, Hansen and Jerome. Veterans of all branches of the military, and all wars and conflicts came out to show their support and enjoy a BBQ lunch complete with distinguished guest Veteran speakers such as Representative Clark Kauffman, Chris Talkington, Lee Heider and the Boise National Guard. We are looking forward to this years’ event which will be on July 15th at Harmon Park in Twin Falls. We were given the honor of coordinating and hosting a private in-home Veterans recognition for one of our patients and his family as well as a multi Veteran recognition at Brookdale Assisted Living Center for two of our patients and several other Veterans residing at Brookdale Assisted Living Center. Our devoted Veteran-to-Veteran Volunteers Tom, Carol and Bob visited 20 Veteran patients in their homes and in facilities and brought them an extra measure of understanding companionship and respect. There’s always room on the Hospice Visions, Inc.’s team of volunteers for more Veterans interested in volunteering for other veterans. Page 4 Flag Dedication Flag Day 2016 In 1916, President Woodrow Wilson issued a proclamation that officially established June 14 as Flag Day; in August 1949, National Flag Day was established by an Act of Congress. Flag Day is not an official federal holiday. Our flag honors those who have fought to protect it, and is a reminder of the sacrifice of our nation's founders and heroes. As the ultimate icon of America's storied history, the Stars and Stripes represent the very best of this nation. Hospice Visions, Inc. once again worked this year with the Snake River Counsel of Boy Scouts of America and helped a local Scout Brycen Funk with his Eagle Project. Brycen and his fellow Scouts of Troop 88 of Kimberly, Idaho design and installed a flag pole. The 25 foot flag pole and concrete base was installed at Rosetta Assisted Living of Twin Falls. Brycen with the help of his family, local contractors and supply stores formed and poured a concrete base to secure the pole in Southern Idaho’s high winds. The Scouts then dug up a flower bed to surround the base and planted flowers. Brycen completed the project in September. Thanks to Scouts Carson Merrick, Samuel Bybee, Justin Bybee, Aiden Bybee, Rex-J Ward, Jencen DeVries, and parents Tim and Susan Funk, Ryan Merrick and Shane Bybee. Veterans at Rosetta Assisted Living can now look out their window and see the stars and stripes every day. Brycen’s wish, along with Hospice Visions, Inc. is to continue to dedicate flag poles at Veterans homes and businesses that support veterans. If you are interested please give us a call. Page 5 Fund raising/Grants Since its inception as a 501(C)(3) non-profit organization in 1995, Hospice Visions, Inc. has worked diligently to provide quality care and comfort for those individuals and their loved ones facing end-of-life concerns throughout Twin Falls, Jerome, Lincoln and Gooding counties regardless of their ability to pay. Through support from community, fund raising, generous donations, and grant opportunities we are able to provide extra supportive services to our patients and families. Grants and Gifts Each year, since 2006, St. Luke’s Magic Valley Medical Center has awarded us funding through their Community Health Improvement Fund. These dollars are used to ensure Hospice Visions, Inc. Can meet the needs of our community by defraying costs and providing care at our “Visions of Home” hospice house. Hospice Visions, Inc. Provides hospice care in this peaceful, quiet, home-like environment to those with limited income as if it were the patient’s own home. Without the help of our community, local businesses and legacy donations, our ability to provide access to quality, compassionate care and supportive services would be impacted. 21st Annual Flo Slatter Memorial Golf Tournament Our “21st Annual Flo Slatter Memorial Golf Tournament” was held on Saturday August 22nd, 2015 at the Clear Lake Country Club in Buhl, Idaho. We had a very successful year with 27 teams participating and over 116 donations from our friends and business community valued at over $8,600.00! There was a delicious trout and buffalo burger lunch served, and no one walked away without a prize from our raffle. Hospice Visions, Inc. Was able to raise over $13,794.00 for our charity care thanks to all the participation from those individuals and businesses who share our vision. Page 6 Fundraising/Grants Art for Hospice “Art for Hospice” was conceived by the local art community to establish a whole new experience in the art of giving. By combining the two, you are given the ability to purchase exquisite art and to support the “Visions of Home” hospice house. As a non-profit organization, Hospice Visions, Inc. “Art for Hospice” is intended to change the paradigm of conventional fundraising by offering the best-of-the-best in art work. This in turn also helps provide the best-of the best in end of life palliative care for our family, friends and neighbors. Art lovers purchased art during the exhibit or commission a work from one of the many artist where 25% of the fee will go to help support Hospice Visions, Inc’s “Visions of Home” hospice house. Our first year at the now annual “Art for Hospice” raised $3899.00 More than 20 artists shared their work, patrons purchased more than $10,736.00 of art to benefit “Visions of Home” hospice house. A very “Special Thank You” to Maria Smith for her guidance and support “Art for Hospice” could not have become a reality without her. Festival of Giving This was our 3rd year participating at the Festival of Giving, sponsored by and held at the Southern Idaho Landscape Center, in November. Dr. Jennifer Glass of West End Veterinary Clinic along with Pat and Liz Warburton of Warburton Courier sponsored Hospice Visions’ Christmas tree again this year. Hospice Visions, Inc. was able to raise more over $4,000.00 from this event to support “Visions of Home” hospice house. This fundraiser was created to help meet the needs of Magic Valley’s non-profit organizations. The Magic Valley Festival of Giving has exceeded their community goal of $120,000!!! Congratulations Magic Valley! Thank you for allowing us to be part of your incredible event. 4th Annual Butterfly Release We delight in the beauty of the butterfly, but rarely admit the changes it has gone through to achieve that beauty. -Maya Angelou We celebrated our 4th Annual Butterfly Release on July 16th, 2016 at Canyon Crest Event Center. The attendance continues to grow as we share this event with family and friends. This year we featured Hospice Visions associates making all the presentations including, Chaplains Dick, Paul and Volunteer Coordinator Nora Wells. Guests collected their live butterflies and then gathered out on the veranda where Chaplain Paul gave us all a blessing and we released hundreds of butterflies into the sunny warm July day. According to folklore, a wish given to a butterfly is always granted, in exchange for setting the butterfly free. We rejoice with those we love and remember the ones we have lost. Proceeds from this event help support our “Visions of Home” hospice house. Thank you Canyon Crest. Page 7 dream foundation Home Safe Home dream foundation® The social work team strives to encourage patients and their families to consider opportunity for growth at the end-of-life. The team has had a year filled with fostering wishes of those receiving care from Hospice Visions, Inc. Hospice Visions, Inc. partners with the Dream Foundation® to provide travel and lodging for families from out-of-town in visiting their loved ones on hospice. We are thankful to the Dream Foundation® for their willingness to assist, within days or hours of a request! Our team helped a veteran war pilot live his wish to have one last flight. Thanks to a generous local pilot, one of our patients was able to fly, and even take the controls. He was so happy he could hardly hold still for days after. We helped raise funds in the community to help a family experience Christmas and Thanksgiving with their loved one, late last summer. A family was given a tree, the trimmings and several hundred dollars for a shopping spree in order to celebrate the holidays. A woman who had never been to the ocean expressed a desire to see a whale before her death. A planned trip to the coast coordinated by the team was canceled due to her poor health, but the team was able to arrange a live streaming video session with Seaworld®. She, her family and some residents in her town were able to have an hour long personal session with trainers and nine whales in her very own home! Hospice Visions, Inc. has a strong desire to assist in the resource development and well-being of our Magic Valley residents. The Home Safe Home program was developed in order to allow professional staff to walk along-side our neighbors, especially when they lack the knowledge or resources to help themselves. The team worked tirelessly to advocate for housing for a man who was forced out of his apartment when it was closed to be remodeled. Arrangements were made for his temporary housing, move, and lodging with advocacy for donations from his community. Families often struggle when it comes to having difficult conversations with their loved ones, especially recognizing the need for additional in-home care or a move to a higher level of care. Through our Home Safe Home program, the social work team at Hospice Visions, Inc. has helped many families facilitate intermediate-to-long-term care planning this year with their aging loved ones in our community. Page 8 Family Comments Every member of the Magic Valley Team did a very good job for my mother. They were all very loving of her, our family and friends.
