 Fill out pages 2 - 4 completely
 Include detailed information
 Include Shipping Address, Contact Information, and Credit Card Billing Information (Include CCV and
Expiry date) with signature
 Include ALL 5 pages inside the package along with your ECU(s)
PACKAGING
 Place completed forms in box
Please Note: When sending multiple ECUs in a single box, a separate form needs to be filled out for
each ECU.
 Wrap ECU in bubble wrap or industrial packing paper and seal box against damage. (Avoid using styro-
foam packing peanuts)

"Car Control Module – Will be Returned to Sender - TEMPORARY EXPORT"
Please Note: When sending in an ECU to avoid heavy duties a value of $10 should be applied to the
customs form. Otherwise, you will be charged duties and import taxes.

I herby agree and understand that chip tuning may involve certain risks, including but not limited to, engine
and/or transmission failure, fire, explosion, personal injuries, death, and/or any vehicle or personal damage that
may occur from the lack of control of the vehicle due to its increased / excessive power or abusive driving.
Installation of performance software has not been approved for street use by any municipal or federal transport
commission and is strictly for OFF-ROAD USE only. I am also aware that this installation may or may not cause
the vehicle to void its dealer warranty and Unitronic will not be held liable. Unitronic Performance Products
come with a 30-day money back satisfaction guarantee. The installation and software are covered by a limited,
non-transferable lifetime warranty that is rendered void upon any evidence of tampering or physical damage to
the ECU. The Client reserves the right to return to OEM (Stock) programming and agrees to incur all costs
involved; including but not limited to additional installation fees. All standard tuning software is developed with
the intent to strictly respect the Canadian and United States emission standards therefore Unitronic offers no
warranty, written or implied, in regards to this matter. Custom or Big Turbo software may not comply with
emission standards and is strictly for OFF-ROAD USE. All Unitronic software has its own unique identification
and encryption. Anyone attempting to decrypt, sell or distribute Unitronic Products without prior written consent,
will be traced and prosecuted. I understand that Unitronic cannot be held liable or responsible.
I also understand and agree that Unitronic cannot be expected to control all of said risks. In consideration of the
benefits, through my participation in this installation, I hereby expressly and knowingly RELEASE UNITRONIC,
ITS OFFICERS, AGENTS, VOLUNTEERS, AND EMPLOYEES FROM ANY AND ALL CLAIMS AND CAUSES
OF ACTION I MAY HAVE FOR PROPERTY DAMAGE, PERSONAL INJURY OR DEATH SUSTAINED BY ME
OR MY VEHICLE BEING CHIP TUNED, CONDUCTED BY, OR UNDER THE AUSPICES OF UNITRONIC,
WHETHER CAUSED BY MY OWN NEGLIGENCE OR THE NEGLIGENCE OF UNITRONIC, ITS OFFICERS,
AGENTS, VOLUNTEERS, OR EMPLOYEES. Furthermore, I voluntarily and knowingly agree to HOLD
HARMLESS, PROTECT, AND INDEMNIFY UNITRONIC, its officers, agents, volunteers, and employees,
against and from any and all claims, demands, or causes of action for property damage, personal injury or
death, including defense costs and attorney’s fees, arising out of my participation in this installation,
REGARDLESS OF WHETHER SUCH DAMAGES, INJURY OR DEATH ARE CAUSED BY MY OWN
NEGLIGENCE, OR BY THE NEGLIGENCE OF UNITRONIC ITS OFFICERS, AGENTS, VOLUNTEERS, OR
EMPLOYEES. I HAVE READ AND UNDERSTOOD THIS DOCUMENT, MY INTENT AND MY CHOICE TO
INSTALL THIS PRODUCT IS A VERBAL AGREEMENT, AND CONFIRMS MY INTENT TO BE BOUND BY ITS
TERMS.
If the participant is under 18, as a parent or guardian, I reflect my agreement to indemnify Unitronic from any
claim which may be brought by or on behalf of the installation, or any member of the participant’s family, for
injury or loss resulting from those inherent risks of the course, described above.
