GOAL/OBJECTIVE/INDICATOR/ACTIVITY/ACTION STEPS/BUDGET
Goal: To educate and connect the healthcare community with diabetes prevention resources.
Objective: Persons who are at-risk for becoming clinically diagnosed as diabetic will be referred into the Y’s
diabetic prevention program by their Primary Care Physician thus receiving the following: 1) specific diabetes
prevention education from diabetes education professionals based upon the principles of Y5210 programming, 2)
additional nutritional education to prevent diabetes from dieticians also based upon the principles of Y5210
programming, 3) additional physical training from licensed personal trainers.
Indicator: We will have 50 persons diagnosed as pre-diabetic referred into the as yet named
diabetes prevention program. These persons will also demonstrate a 5% reduction in their
bodyweight as a result of changing their behaviors to a healthier lifestyle as a result of their
diabetes prevention education based upon Y5210 princples.

Referrals from local primary care providers will receive diabetes prevention education from
local professionals (Dieticians and Certified Personal trainers) who specialize in diabetes
education. Diabetes education will include nutrition/education to prevent diabetes and
personal training to physically change their habits. Referrals include memberships to the Y,
personal training, and diabetes nutritional education/screening.
Promotional dinner for local
providers
To inform local healthcare providers of our diabetes prevention programming and the
referral process.

Goal: Raise awareness and participation in the Y5210 program and principles in conjunction with Coordinated

School Health.
Objective: Students ages Pre-K through 12th grade will make informed choices to have a healthier diet and
physical lifestyle.
Indicator: Fifty percent of students participating will demonstrate an adherence to a minimum of
two of the healthy living principles of the Y5210 Program: 1) eating five or more servings of fruits
and vegetables; 2) watching less than two hours of screen time; 3) having one hour or more of
active play: and 4) drink no sugary drinks.

Y5210 program
Action Steps / Date
Students who are eligible will participate in the 10 week long Y5210 program. Students will be
deemed eligible with a pre-program weigh-in for BMI’s; progress will also be measured through
a post-program weigh-in to gather BMI’s. This program includes a pre and post survey to assess
participants’ knowledge and ability to adhere to the healthy living principles of the Y5210
program.

Goal: To empower low income families to adopt a healthy lifestyle.
Objective: Families will participate in the community garden program and learn how to grow, tend, harvest, and
prepare fresh fruits and vegetables.
Indicator: Ninety percent of all garden beds available will be utilized by individuals/families.

Community Gardens
Participation in the community garden program. This program includes gardening classes including
the following topics: what to plant, how to plant, how to tend, and how to harvest. There will also
be additional educational opportunities provided to these participants via the University of
Tennessee Ag Extension which will cover food preparation and food storage.
Rails to Trails project
To provide matching funds to the Rails to Trails project in getting the infrastructure in place for the
walking trail.

Goal: Provide Chronic Disease Self-Management Programming (CDSMP) to the community.
Objective: To Educate persons diagnosed diabetic or “at-risk” of to become diabetic educational
opportunities to manage their disease.
Indicator: To have a minimum of 85% participation rate for classes offered through the University
of Tennessee Ag Extension and through Starr Regional Medical Center.
