The Flow Diary - Stop Bedwetting

THE
FLOW-DIARY
Patient Information Leaflet
Sponsored by:
You have been sent an appointment to see the
Doctor/Nurse about your bedwetting and have
been asked to complete a bladder diary prior to
your appointment.
Why do I need to complete a bladder diary?
A diary is a useful way to understand how often the
bedwetting happens and if your bladder is working as
it should during the day or night.
The information from this diary is very important in helping
your Doctor/Nurse to understand your symptoms clearly so
they can provide the best possible advice and treatment.
Options for diary completion
You can complete the bladder diary online using the Flow
Diary, available via desktop, tablet or mobile devices. If you
do not have online access, please complete the enclosed
paper version.
There is very little information around bedwetting and
the effects this can have on an individual so your input
is invaluable in helping clinicians and researchers gather
evidence to support future
improvement and funding
to the care in this area.
By completing this
online version you will be
contributing to help your
Doctor/Nurse with this
important data collection.
How often do I need to input
information?
The diary information should be started at least 7 days before
your first appointment with your Doctor/Nurse. Following
a clinical review at your appointment, you may be asked to
continue to complete your diary for a period of time.
If you use the Flow Diary, there is a simple questionnaire that
will need to be completed prior to your first appointment.
You may also be prompted to repeat the questionnaire at set
intervals whilst under the care of the Doctor/Nurse.
How do I register to use the
online diary?
To register, enter the link below into any web browser:
https://www.sciensusapps.com/flowdiary
If you have already discussed the online diary with a Doctor/
Nurse, you may have already been registered using your email
address. If so, you should follow the instructions that will have
been emailed to you. If you have not been pre-registered, you
can self-register via the “registered?” link on the log in page.
Once registered, your Doctor/Nurse will be able to access the
information you input online and also when you are reviewed in
the clinic. This will help them to monitor your progress.
How do I get started?
Once registered you will be guided through a tutorial
on how to use the Flow Diary application and be asked to
complete a simple questionnaire around how you feel about
your condition.
Will my information be kept secure
and what will it be used for?
Your data will be held securely within our application and will
only be shared with your Doctor/Nurse in charge of your care.
Your data may be used in anonymised form to support research
and publications within this treatment area.
As part of the service, we are collecting information as to why
patients may not attend scheduled appointments. Should you
not attend an appointment you may be contacted by a Nurse
to identify the reason why.
For more information please contact your Doctor/Nurse in
charge of your care.
