This form is used to apply for admission to the Associate of Science Nursing Program. Students interested in the program must first
apply for admission to the College and are admitted as Biology-Healthcare majors. Admission as a Biology-Healthcare major does not
guarantee admission to the Associate of Science Nursing Program. Each fall semester, forty students are admitted to the program. If
not admitted, applicants must reapply each year.

APPLICATION INSTRUCTIONS
You must read and complete all of the steps to ensure a complete application. Only complete applications will be reviewed.
Applications for the fall 2017 program start are accepted beginning January 1, 2017 through the application deadline March 1st,
2017. No applications will be accepted after the deadline. It is STRONGLY RECOMMENDED that you complete the following steps at
least a couple months prior to the Nursing program deadline in March to ensure your transcripts have been received and that your
coursework has been evaluated in time to submit this application for the Associate of Science Nursing Program.
STEP 1: COMPLETE ITEMS PRIOR TO PROGRAM APPLICATION SUBMISSION
1) Admitted as a CWI Student - https://forms.cwidaho.cc/registration/app01.php
2) Complete the Bachelor’s Degree Evaluation Request form if you have completed a bachelor’s or master’s degree.
(Do not include the Bachelor’s Degree Evaluation Request form with your nursing application. Please submit to One Stop.)
3) TEAS Assessment - Please go to the following link http://cwidaho.cc/current-students/assessment-and-testing-centers to
schedule your TEAS assessment WELL in advance of submitting your nursing application. Openings are on a first-come,
first-serve basis and fill up fast! Information and study guides are available online at: www.atitesting.com. You can take
the TEAS as many times as you want. It is your responsibility to include a copy of your TEAS assessment in your application
documentation upon submitting your application. TEAS scores are good for up to five years.
STEP 2: REQUIRED APPLICATION SUBMISSION MATERIALS
1) AS Nursing Program Application (this form) - Only complete applications will be considered
2) Enclose a copy of your Biology Healthcare AS Program Evaluation, for your specific catalog year, which you will be utilizing
to score this nursing application. (If you are unsure of your catalog year, please see your student profile in your MyCWI)
3) Copy of current healthcare certification (if applicable)
4) Copy of TEAS Assessment (must be within 5 years)
STEP 3: CALCULATE CUMULATIVE GPA FOR THE FOLLOWING COURSES WITH A “C” OR HIGHER for a total of eight points
Completion of the math and science courses is not required to apply to the Associate of Science Nursing Program. However,
program admission is competitive so completion of these courses increases your ranking points. These courses must be completed
prior to application if you expect to obtain completion points. In-progress courses cannot be used because you do not have a final
grade in the course. Completion of these courses and/or degrees must be part of your CWI student record. Course equivalencies
are determined by CWI.

While a healthcare certification is not required to apply, they do count towards your ranking points. Include a copy of your current
certifications. Expired or lapsed certifications will not be accepted. You must use the certificate with the highest points. Ex: If you
have a CMA and an EMT certification, you will score a “2” not a “3”.
