(CTALD) is made up of Health and
Social Services workers
They can give help and advice to
people with learning disabilities and
their carers living in Somerset
CTALDs can support you to choose
How you would like
to live your life
What you want to
achieve
How to stay safe
CTALDs operate a
referral system
This means that
people who know
you, like your
family or doctor,
can ask for help
Or, you can ask
for help in writing
or by phone
A member of our team
will come and visit you
and talk about what you
need help with
This is written down and
is called an assessment
We will decide if we
can help
We will talk with you
about who might help
We might check to see
if you need to pay for
some services
